摘要
目的对该院第二类精神药品进行处方点评,评价不合格处方情况。方法:研究时间以2018年1月—2019年4月为准,回顾此期间该院应用第二类精神药品的4 714例患者资料,根据处方点评结果,分析不合格处方的原因。结果该院2018年1月—2019年4月期间总计使用15种第二类精神药品,其中片剂有11种,注射剂有4种,处方最多的是苯巴比妥钠注射液,最少的是佐匹克隆片。4 714张第二类精神药品处方中,统计到不合格处方108张,不合格率为2.29%;这些处方中82例患者发生不良反应,不良反应发生率高达1.74%,占不合格处方的75.93%;主要为消化系统、中枢神经系统、泌尿系统和心脑血管系统方面的不良反应。临床诊断不明确、适应证不适宜、用法用量不当、联合用药不当是引起处方不合格的主要原因。结论该院2018年1月—2019年4月第二类精神药品的使用还存在一定的不合理之处,要确保药品的使用安全,还应加强对精神药品的管理,提高药师的专业技能。
Objective To make a prescription review of the second class of psychotropic drugs in the hospital and evaluate the unqualified prescriptions. Methods The study time was based on January 2018 to April 2019, and the data of 4 714 patients who applied the second class of psychotropic drugs in our hospital during this period were reviewed. The reasons for the unqualified prescriptions were analyzed according to the results of the prescription reviews. Results A total of 15 second-class psychotropic drugs were used in the hospital from January 20 to April 2019, including 11 tablets and 4 injections. The most prescribed drug was phenobarbital sodium injection, the least. It is a zopiclone tablet. Among the 4 714 prescriptions for the second class of psychotropic drugs, 108 were unqualified prescriptions, and the unqualified rate was 2.29%. Among these prescriptions, 82 patients had adverse reactions, and the incidence of adverse reactions was as high as 1.74%, accounting for 75.93% of the unqualified prescriptions. Mainly for adverse reactions in the digestive system, central nervous system, urinary system and cardiovascular and cerebrovascular system. Unclear clinical diagnosis, improper indications, improper use and dosage, and improper combination of drugs are the main reasons for the unqualified prescription. Conclusion There is still some unreasonable use of the second class of psychotropic drugs in the hospital from January 2018 to April 2019. To ensure the safety of drug use, the hospital should strengthen the management of psychotropic drugs and improve the professional skills of pharmacists.
