摘要
目的探讨异常子宫出血的常见原因以及宫腔镜的诊治价值。方法选择2015年1月—2018年12月就诊于该院的88例异常子宫出血为研究对象,分析引起患者出血的原因,并行宫腔镜检查,以病理诊断结果为金标准,分析宫腔镜检出情况,评估宫腔镜的治疗效果。结果异常子宫出血主要因素是子宫内膜息肉,占52.27%,其次为功能性子宫出血、子宫黏膜下肌瘤均占17.04%,15.90%;以病理诊断结果为金标准,宫腔镜对功能性子宫出血、子宫内膜息肉、子宫黏膜下肌瘤、瘢痕憩室、子宫内膜癌、宫颈残留、子宫内膜炎检出率比较,差异无统计学意义(P>0.05);宫腔镜治疗异常子宫出血的总体治愈率为85.23%,功能性子宫出血、子宫内膜息肉、子宫黏膜下肌瘤、瘢痕憩室治愈率分别为86.66%、93.47%、85.71%、66.67%。结论子宫内膜息肉、功能性子宫出血、子宫黏膜下肌瘤是异常子宫出血的常见原因,宫腔镜对异常子宫出血诊断准确率、治愈率较高,可将其作为诊治异常子宫出血的首选方式。
Objective To investigate the common causes of abnormal uterine bleeding and the value of hysteroscopy. Methods 88 patients with abnormal uterine bleeding who were admitted to our hospital from January 2015 to December 2018 were selected as subjects. The causes of bleeding were analyzed. Parallel hysteroscopy was performed. The pathological diagnosis was used as the gold standard to analyze hysteroscopy. Check the condition and evaluate the therapeutic effect of hysteroscopy. Results The main factors of abnormal uterine bleeding were endometrial polyps, accounting for 52.27%, followed by functional uterine bleeding and uterine submucosal fibroids, for 17.04%,15.90%;pathological diagnosis was the gold standard, hysteroscopy for functional uterine bleeding endometrial polyps, uterine submucosal fibroids, keloid sac, endometrial cancer, cervical residual, endometritis detection rate was compared, the difference was not statistically significant (P>0.05);hysteroscopic treatment of abnormal uterus overall cure rate of bleeding was 85.23%. The cure rates of functional uterine bleeding, endometrial polyps, uterine submucosal fibroids and keloids were 86.66%, 93.47%, 85.71% and 66.67%, respectively. Conclusion Endometrial polyps, functional uterine bleeding and uterine submucosal fibroids are common causes of abnormal uterine bleeding. Hysteroscopy has a high diagnostic accuracy and cure rate for abnormal uterine bleeding, which can be used as the first choice for the diagnosis and treatment of abnormal uterine bleeding.
