摘要
目的:探讨实时剪切波弹性成像(SWE)对子宫颈癌前病变及子宫颈癌的诊断价值。方法:筛选120例成年已婚女性运用经阴道SWE进行诊断,并记录其杨氏模量最大值(Emax)及平均值(Emean)。其中病理确诊为子宫颈鳞癌患者60例(A组),子宫颈上皮内瘤变患者30例(B组)以及正常对照组30例(C组)。绘制Emax及Emean诊断子宫颈病变的受试者ROC曲线,确定其截断点并计算敏感性、特异性、准确性、阳性预测值、阴性预测值及曲线下面积。结果:A、B、C三组Emax、Emean分别为(186.35±34.19)kPavs(97.57±17.15)kPavs(82.01±16.58)kPa、(178.49+32.09)kPavs(90.50±16.55)kPavs(76.04±15.29)kPa,各组之间比较差异均具有统计学意义(P<0.05)。当Emax及Emean临界值取100.8kPa及95.7kPa,敏感度均为81.11%、特异度均为90.01%,准确度均为66.67%,阳性预测值均为96.05%、阴性预测值均为61.36%,AUC分别为0.919、0.913。结论:SWE对诊断子宫颈癌前病变及子宫颈癌有较高的价值。
Objective:To evaluate the diagnostic value of real-time shear wave elastography(SWE) in cervical precancerous lesions and cervical cancer.Methods:Transvaginal SWE was used to screen 120 adult married women.The maximum Young's modulus(Emax) and the mean value(Emean) were recorded.There were 60 cases of cervical squamous cell carcinoma(group A) confirmed by pathological examination,30 cases of precancerous lesion(group B) and 30 cases of normal control group(group C).The ROC curves of Emax and Emean for the diagnosis of cervical lesions were draw.Thetruncation points were determined and the sensitivity,specificity,accuracy,positive predictive value,negative predictive value and area under the curve were calculated. Results:The Emaxin the three groups were(186.35±34.19) kPa,(97.57±17.15) kPa and(82.01±16.58) kPa,respectivaly. The Emean in the three groups were(178.49±32.09) kPa,(90.50±16.55) kPa and(76.04±15.29) kPa,respectively.There was significant difference between each group(P< 0.05).When the critical values of Emax and Emean were 100.8 kPa and 95.7 kPa,the sensitivity was 81.11%,the specificity was 90.01%,the accuracy was 66.67%,the positive predictive value was 96.05% and the negative predictive value was 61.36%.AUC was 0.919、0.913,respectively.Conclusions:SWE has a high value in the diagnosis of cervical precancerous lesions and cervical cancer.
