摘要
目的:探讨体外受精-胚胎移植(IVF-ET)周期中获卵数<15个的多囊卵巢综合征(PCOS)患者采用鲜胚移植与全胚冷冻方案后的妊娠情况。方法:回顾性分析2014年1月至2018年12月在我院接受IVF治疗获卵数<15个的PCOS患者共441例,根据胚胎移植方案分为两组,鲜胚移植方案组(A组,313例),全胚冷冻方案组(B组,128例),比较两组患者的妊娠情况。结果:B组扳机日的血清孕酮(P)水平显著高于A组(P<0.05),两组囊胚移植率差异无统计学意义(P>0.05)。A组患者的累计活产率显著高于B组(72.84%vs63.28%,P<0.05),两组患者第一周期(A组鲜胚移植,B组第一次冻融胚胎移植周期)移植的活产率(47.60%vs51.56%,P>0.05)及胚胎着床率差异无统计学意义(46.00%±33.09%vs49.08%±36.80%,P>0.05)。结论:对于低获卵数(<15个)的PCOS患者,鲜胚移植较全胚冷冻方案能获得更高的累计活产率。
Objective:To compare the pregnancy rate between fresh embryos transfer and freeze-all policy in women with polycystic ovary syndrome who retrieved oocytes less than 15 in IVF-ET cycle.Methods:This retrospective study conducted from January 2014 to December 2018.A total of 441 patients with PCOS experienced IVF-ET were included.The number of oocytes retrieved was less than 15.The patients were divided into the fresh embryos policy group(A group,313 cases) and freeze-all embryos policy group(B group,128 cases) according to different protocols.The cumulative live-birth rate and implantation rate of patients in the two groups were compared.Results:The serum level of progesterone in group B was significantly higher than that in group A on hCG day(P<0.05).There was no significant difference in blastocyst transfer rate between the two group(P>0.05).The rate of cumulative live-birth was significantly higher in group A(72.84% vs 63.28%,P< 0.05).There was no significant difference inthe live-birth rate for the first transfer cycle(47.60% vs 51.56%,P>0.05) or implantation rate(46.00%±33.09% vs 49.08%±36.80%,P>0.05).Conclusions:Fresh embryos transfer might be the preferred protocol for the PCOS patients with the number of oocytes retrieved less than 15 than freeze-all policy.
