辅助性Th17细胞在HPV感染宫颈病变患者外周血的比例及临床意义 被引量:9

摘要 目的探讨辅助性Th17细胞在人乳头状瘤病毒(HPV)感染宫颈病变患者外周血的比例及作用机制。方法将2016年1月-2018年1月间在医院就诊的151例宫颈病变HPV感染患者根据病变类型分为慢性宫颈炎(CCS)组42例、宫颈上皮内瘤变(CIN)组51例、宫颈癌(CC)组58例。检测三组HPV分型,比较不同宫颈病变类型患者以及不同HPV感染类型患者外周血辅助性Th17细胞比例及白介素-17(IL-17)、白介素-23(IL-23)水平,并比较不同临床特征的CC患者外周血Th17细胞比例。结果 CCS组、CIN组和CC组Th17细胞比例、IL-17、IL-23水平以及高危型HPV感染发生率均依次升高(P<0.05)。高危型HPV感染患者外周血Th17细胞比例及IL-17、IL-23水平均明显高于低危型HPV感染患者(P<0.05)。CC患者外周血Th17细胞比例与FIGO分期和分化程度有关(P<0.05)。存在脉管浸润、淋巴结转移的CC患者外周血Th17细胞比例均明显高于无脉管浸润、无淋巴结转移的CC患者(P<0.05)。结论随着宫颈病变程度的恶化,外周血辅助性Th17细胞比例升高。Th17细胞可能参与高危型HPV感染引导的免疫逃逸过程而促进CC的发生、发展。 OBJECTIVE To explore the percentage of helper Th17 cells in peripheral blood of patients with cervical lesions human papillomavirus(HPV) and observe its action mechanisms. METHODS A total of 151 patients with cervical lesions HPV infection who were treated in the hospital from Jan 2016 to Jan 2018 were enrolled in the study and divided into the chronic cervicitis(CCS) group with 42 cases, the cervical intraepithelial neoplasia(CIN) group with 51 cases and the cervical cancer(CC)group with 58 cases according to the types of lesions. The types of HPV were detected, the percentage of peripheral blood helper Th 17 cells and levels of interleukin-17(IL-17) and interleukin-23(IL-23) were compared among the three groups of patients and among the patients with different types of HPV infection, and the percentage of peripheral blood helper Th17 cells among the patients with CC. RESULTS The percentage of Th17 cells and levels of IL-17 and IL-23 as well as the incidence of high-risk HPV infection were successively elevated in the CCS group, the CIN group and the CC group(P<0.05);the percentage of peripheral blood Th17 cells and levels of IL-17 and IL-23 of the patients with high-risk HPV infection were significantly lower than those of the patients with low-risk HPV infection(P<0.05). The percentage of peripheral blood Th17 cells of the CC patients was associated with the FIGO stage and differentiation degree(P<0.05). The percentage of peripheral blood Th17 cells of the CC patients who had vascular infiltration and lymph node metastasis was significantly higher than that of the CC patients who did not have vascular infiltration and lymph node metastasis( P<0.05). CONCLUSION With the deterioration of the degree of cervical lesions, the percentage of peripheral blood helper Th cells is elevated. Th17 cells may participate in the process of immune escape induced by high risk HPV infection and promote the occurrence and development of CC.
