摘要
目的分析LASIK术后外伤性角膜瓣移位的表现、处理及结果。方法对本院8年间7例(7眼)的临床资料进行回顾性分析。结果角膜瓣移位分别发生在术后2周、1个月、1.5个月、6个月、2年、5年、10年各1例,6例伤后12小时内复位者,术后1周裸眼视力≥0.6,术后1个月≥0.8;1例伤后1周复位者,术后1周裸眼视力0.5,术后1个月视力0.6+2。所有患者术后角膜瓣对位良好,5例角膜透明,无上皮植入内生等并发症。1例瓣边缘少量局限性点状上皮植入,1例中央基质浅层轻度浑浊,瓣边缘环形灰白色上皮植入,观察6个月均无发展趋势。结论LASIK术后角膜瓣并不能达到真正的愈合,术后需避免眼外伤,伤后及时处理是恢复视力的主要手段。
Objective To analyse the clinical manifestation,treatment and results of traumatic corneal flap displacement after LASIK.Methods The data of 7 eyes of 7 cases of traumatic corneal flap displacement after LASIK from Jun.2009 to May 2017 were analyzed retrospectively.Results The displacement of corneal flaps occurred in 1 case each at 2 weeks,1 month,1.5 months,6 months,2 years,5 years and 10 years after operation respectively.The uncorrected visual acuity of 6 cases whose corneal flap relocation within 12 hours after injury was≥0.6 at 1 week after surgery,≥0.8 in 1 month after surgery.One case whose corneal flap relocation at 1 week after injury,and the visual acuity was 0.5 at 1 week after surgery and 0.6+2 at 1 month after operation.The corneal flaps were well aligned in all patients,and there were 5 cases with transparent cornea.No complication such as epithelial implantation or endogenesis happened.A small amount of localized punctate epithelium was implanted at the edge of the corneal flap in 1 case.The superficial layer of the central stroma was slightly turbid,and the annular grey-white epithelium was implanted at the edge of the corneal flap in 1 case.There was no trend in 6 months.Conclusion After LASIK the corneal flap cannot achieve true healing,patients should avoid ocular trauma after surgery.Timely and proper treatment after injury is the main means to restore vision.
