摘要
目的研究何氏骨科外敷中药逐阴散治疗膝关节滑膜炎的临床疗效。方法收集膝关节滑膜炎患者86例,随机分为试验组与对照组,各43例。试验组接受外敷中药逐阴散治疗,对照组口服滑膜炎片治疗,比较两组疗效、膝关节疼痛评分、血清和关节液瘦素水平、滑膜厚度、关节腔积液、膝关节功能评分、关节液炎症因子[肿瘤坏死因子(TNF-α)、白细胞介素-1(IL-1)、IL-6]和不良反应。结果试验组治疗总有效率为93.02%,高于对照组的76.74%(P<0.05);治疗后,试验组膝关节疼痛评分、血清和关节液瘦素水平、膝关节液TNF-α、IL-1、IL-6水平均低于对照组,滑膜厚度、关节腔积液少于对照组,膝关节功能评分均高于对照组(P<0.05);两组均无明显不良反应发生。结论逐阴散治疗膝关节滑膜炎效果确切,能够减轻炎症反应和关节积液,改善关节功能,其作用可能与降低瘦素水平有关,值得推广。
Objective To study the clinical efficacy of TCM Zhuyin Powder made based on the He's orthopedics theory in the treatment of gonarthromeningitis by external application. Methods A total of 86 gonarthromeningitis patients were selected and randomly divided into an experimental group and a control group, 43 patients for each group. The Zhuyin Powder was applied externally on the patients in the experimental group, and the gonarthromeningitis tablets were performed on the control group. The efficacy, knee joint pain score, serum and joint fluid leptin level, synovial membrane thickness, joint cavity effusion, knee joint function score, inflammatory factor of joint fluid [tumor necrosis factor (TNF-α), interleukin-1 (IL-1) and IL-6] and adverse reaction were compared. Results The total efficacy rate in the experimental group was 93.02%, higher than that in the control group (76.74%)(P < 0.05);after the treatment, the experimental group was lower than the control group in knee joint pain score, serum and joint fluid leptin level, TNF-α, IL-1 and IL-6 level, synovial membrane thickness, and joint cavity effusion, but higher in knee joint function score (P < 0.05). No significant adverse reaction was observed in both groups. Conclusion Possibly by lowering the leptin level, the Zhuyin Powder is efficacious in treating gonarthromeningitis, which can be told from the reduced inflammatory reaction and joint cavity effusion, and improved knee joint function. Hence, it is worth promoting.
