摘要
目的根据2013-2016年兰州市糖尿病视网膜病变(DR)患者住院费用,运用通径分析、灰色关联度等方法进行分析,探索各个影响因素的内在联系,为减轻糖尿病患者经济负担、制定相关改革政策提供理论支持和可靠依据。方法使用通径分析与灰色关联度得出影响DR患者住院费用的最大影响因素与住院总费用各个部分影响总费用的程度。结果对住院费用影响较大的因素包括住院天数、城镇职工医疗保险和职业,影响总效应分别为0.328、0.266和-0.185,住院总费用的增长与服务费、药费和治疗费密切相关。结论医院应尽量减少轻度患者住院日,社区及时对重度患者进行转诊治疗,政府应加快医疗付款方式改革,患者做好预防工作,降低DR发病率。
OBJECTIVE According to the hospitalization expenses of patients with diabetic retinopathy in Lanzhou from 2013 to 2016,the path analysis and grey correlation degree were used to explore the intrinsic connection of various influencing factors,provide theoretical support and reliable basis for reducing the economic burden of diabetic patients and formulate relevant reform policies. METHODS Using the correlation analysis and the gray correlation degree,the maximum influencing factors affecting the hospitalization expenses of DR patients and the total cost of hospitalization affected by the total cost of hospitalization were analyzed. RESULTS The main factors affecting hospitalization expenses,including hospitalization days,urban employee medical insurance and staff were 0.328,0.266 and-0.185.The increase in the total cost of hospitalization was closely related to the service fee,drug fee and treatment fee. CONCLUSION The hospital should minimize the hospitalization day for mild patients,the community and the referral treatment for severe patients. The government speed up the reform of medical payment methods,and patients should prevent and reduce the incidence of DR.
