摘要
目的:在脑胶质瘤诊断及分级中应用多层螺旋CT灌注成像,探讨其应用价值,为日后临床相关工作提供借鉴。方法:选取2017年1月16日-2018年5月10日本院收治的脑胶质瘤患者62例。以WTO发布神经系统肿瘤分类标准作为依据,结合病理结果,将患者分为低度恶性组(n=18)、中度恶性组(n=32)、高度恶性组(n=12)。三组均实施多层螺旋CT灌注成像诊断,对比各组肿瘤实质区、正常脑组织区各项血流指标(PS、BF、BV、rBV、rBF)。结果:瘤体实质区PS、BF、BV均高于正常脑组织(P<0.05);低度恶性组瘤体实质区PS、BF、BV均低于中度恶性组、高度恶性组,且中度恶性组瘤体实质区PS、BF、BV均低于高度恶性组,差异均有统计学意义(P<0.05);三组对侧正常脑组织区PS、BF、BV比较,差异均无统计学意义(P>0.05);低度恶性组rBV、rBF均低于中度恶性组、高度恶性组,且中度恶性组rBV、rBF均低于高度恶性组,差异均有统计学意义(P<0.05)。结论:在脑胶质瘤诊断及分级中,应用多层螺旋CT灌注成像,可清楚地了解患者的肿瘤实质区血流变化情况,为日后临床治疗提供参考依据。
Objective:To explore the application value of multi-slice spiral CT perfusion imaging in the diagnosis and grading of glioma,and to provide reference for future clinical work.Method:A total of 62 patients with glioma from January 16,2017 to May 10,2018 were selected.Based on the classification criteria of neurological tumors issued by WTO and combined with pathological findings,the patients were divided into low malignant group(n=18),moderate malignant group(n=32)and high malignant group(n=12).Multi-slice spiral CT perfusion imaging was performed in three groups,and the blood flow indices(PS,BF,BV,rBV,rBF)of tumor parenchymal area and normal brain tissue area in each group were compared.Result:The PS,BF and BV in tumor parenchymal area were higher than those of normal brain tissues area(P<0.05).The PS,BF and BV in tumor parenchymal area of low malignant group were lower than those of moderate malignant group and high malignant group,and the PS,BF and BV in tumor parenchymal area of moderate malignant group were lower than those of high malignant group,the differences were statistically significant(P<0.05).The PS,BF and BV of normal brain tissue area in three groups were compared,the differences were not statistically significant(P>0.05).The rBV and rBF in low malignant group were lower than those of moderate malignant group and high malignant group,and the rBV and rBF in moderate malignant group were lower than those of high malignant group,the differences were statistically significant(P<0.05).Conclusion:In the diagnosis and grading of glioma,multislice spiral CT perfusion imaging can clearly understand the changes of blood flow in the tumor parenchyma,and provide a strong reference for future clinical treatment.
