摘要
目的:比较分析双endobutton钢板及锁骨钩钢板治疗肩锁关节脱位的临床效果。方法:选取2017年1月至12月东莞医院收治的98例肩锁关节脱位患者为研究对象,随机分为观察组和对照组,各49例。观察组采用双endobutton钢板,对照组采用锁骨钩钢板治疗,比较2组患者围术期指标、术后12个月疼痛VAS评分、肩关节功能Constant评分及肩锁关节间隙。结果:对照组患者术中出血量、手术时间明显高于观察组(P<0.05);2组患者的切口长度的比较,差异无统计学意义(P>0.05);2组术前与术后12个月的肩关节功能Constant评分和疼痛VAS评分比较,差异均无统计学意义(P>0.05);2组术后12个月肩关节功能Constant评分和疼痛VAS评分与本组术前比较,差异均有统计学意义(P<0.05)。对照组复位丢失量明显多于观察组(P<0.05);2组患者并发症发生率比较,差异无统计学意义(P>0.05)。结论:肩锁关节脱位患者采取双endobutton钢板及锁骨钩钢板治疗均获得良好的临床效果,但双endobutton钢板手术时间、术中出血量及复位丢失量效果更优,有利于提升患者康复水平。
Objective: To compare and analyze the clinical effects of double endobutton plate and clavicular hook plate in the treatment of acromioclavicular joint dislocation. Methods:A total of 98 patients with acromioclavicular joint dislocation admitted to our hospital from Jan 2017 to Dec 2017 were enrolled. The patients were randomly divided into observation group and control group, 49 cases in each group. The observation group was treated with double endobutton plate and the control group was treated with clavicular hook plate. The perioperative indexes, VAS score, shoulder joint function (Constant score) and acromioclavicular joint space of the two groups were compared. Results: The intraoperative bleeding and operation time of patients in the control group were significantly higher than those in the observation group(P<0.05). There was no significant difference in the incision length between the two groups(P >0.05). There was no significant difference in the Constant score of shoulder joint function and VAS score before and 12 months after operation between the two groups(P>0.05). At 12 months after operation, there were significant differences in Constant score of shoulder joint function and VAS score of the two groups compared with those before operation (P<0.05). The reduction loss in the control group was significantly more than that in the observation group (P<0.05).There was no significant difference in the incidence of complications between the two groups (P>0.05). Conclusion: Double endobutton plate and clavicular hook plate are effective in the treatment of acromioclavicular joint dislocation, but the operation time, intraoperative bleeding and reduction loss of double endobutton plate are better, which is conducive to improving the rehabilitation level of patients.
