摘要
目的对孕妇牙周炎与不良妊娠结局关系展开前瞻性研究。方法方便选取2017年9月—2018年3月118例患有牙周炎的孕妇和118名未患牙周炎孕妇作为研究对象,将患牙周炎孕妇设为研讨组,未患牙周炎设为参照组,对两组患者的不良妊娠结局进行观察对比,并对其孕妇牙周炎与不良妊娠结局关系进行相关性分析。结果参照组早产、流产、子痫前期、胎膜早破、低出生体重、妊娠期高血压及早产低出生体重等不良妊娠结局发生率分别为0.85%、4.24%、3.39%、5.08%、1.69%、5.08%、0.85%,研讨组的不良结局发生率分别为6.78%、2.54%、1.69%、1.69%、7.63%、1.69%、6.78%。通过单因素分析发现,两组患者流产(P=0.47)、子痫前期(P=0.41)、胎膜早破(P=0.15)及妊娠期高血压(P=0.15)的发生率差异无统计学意义(P>0.05);研讨组的早产(P=0.02)、低出生体重(P=0.03)及早产低出生体重(P=0.02)的发生率均显著高于参照组(P<0.05)。通过多因素Logistic回归分析发现,孕妇牙周炎为影响早产、低出生体重及早产低出生体重等不良妊娠结局的独立危险因素。结论孕妇牙周炎与流产、子痫前期、胎膜早破及妊娠期高血压等不良妊娠结局无相关性,与早产、低出生体重及早产低出生体重等不良妊娠结局具有相关性,且为独立危险因素。
Objective To prospectively study the relationship between maternal periodontitis and adverse pregnancy outcomes.Methods From September 2017 to March 2018,118 pregnant women with periodontitis and 118 pregnant women without periodontitis were convenient selected as the study subjects.Pregnant women with periodontitis were selected as the study group.The adverse pregnancy outcomes of the two groups were observed and compared,and the relationship between maternal periodontitis and adverse pregnancy outcomes was analyzed.Results The incidences of adverse pregnancy outcomes such as preterm birth,abortion,preeclampsia,premature rupture of membranes,low birth weight,gestational hypertension and preterm low birth weight were 0.85%,4.24%,3.39%,5.08%,1.69%,5.08%,and 0.85%,the incidence of adverse outcomes in the study group were 6.78%,2.54%,1.69%,1.69%,7.63%,1.69%,and 6.78%,respectively.Univariate analysis showed that there was no statistically significant difference in the incidence of abortion(P=0.47),preeclampsia(P=0.41),premature rupture(P=0.15),and gestational hypertension(P=0.15)(P>0.05).The incidence of preterm birth(P=0.02),low birth weight(P=0.03),and preterm low birth weight(P=0.02)were significantly higher in the study group than in the control group(P<0.05).Multivariate logistic regression analysis found that maternal periodontitis was an independent risk factor for adverse pregnancy outcomes such as preterm birth,low birth weight and preterm low birth weight.Conclusion There is no correlation between maternal periodontitis and adverse pregnancy outcomes such as abortion,preeclampsia,premature rupture and pregnancy-induced hypertension.It is associated with adverse pregnancy outcomes such as premature delivery,low birth weight and preterm low birth weight,which were risk factors.
