摘要
目的观察引导式教育对不随意运动型脑瘫患儿运动及认知功能的影响。方法选取2017年1-12月在广州中医药大学附属南海妇产儿童医院康复科就诊的60例不随意运动型脑瘫患儿作为研究对象,按就诊先后顺序分为对照组和治疗组,各30例。对照组给予常规康复训练,治疗组在对照组治疗方法的基础上给予引导式教育(CE),2组均治疗3个疗程后,观察粗大运动功能分级系统(Gross Motor Function Classification System,GMFCS)分级和北京盖什尔发育量表(Beijing Gesell Developmental Scale,Gesell)的发育商(DQ)评分变化情况。结果治疗前,2组GMFCS分级比较,差异均无统计学意义(P>0.05),具有可比性;治疗后,治疗组GMFCS分级较治疗前提高,差异均有统计学意义(P<0.05),且治疗组Ⅴ级优于对照组,差异有统计学意义(P<0.05)。治疗前,2组社会适应、个人-社会、大运动、精细运动和语言的DQ评分比较,差异均无统计学意义(P>0.05),具有可比性;治疗后,2组上述评分较同组治疗前均有所提高(P<0.05或P<0.01),且治疗组升幅优于对照组,差异均有统计学意义(P<0.01)。结论 CE能促进不随意运动脑瘫患儿运动、认知功能的提高,通过家长的全程参与,为患儿的家庭康复奠定了一定基础,值得临床进一步探索应用。
Objective To observe the effects of conductive education (CE) on motor and cognitive function in children with dyskinetic cerebral palsy.Methods Sixty cases of dyskinetic cerebral palsy consulted in rehabilitation department of Nanhai Women’s and Children’s Hospital Affiliated to Guangzhou University of Chinese Medicine from January to December in 2017 were chosen as the object of study and divided into control group and treatment group according to the order of visiting time,with 30 cases in each.The control group was given routine rehabilitation training,while the treatment group was given CE on the basis of the therapeutic method of the control group. After the 2 groups were both treated for 3 courses,the changes of Gross Motor Function Classification System (GMFCS)and developmental quotient (DQ) of Beijing Gesell Developmental Scale (Gesell) were observed.Results Before treatment,there was no statistically significant difference in grading comparison of GMFCS between the 2 groups (P>0.05),and they were comparable.After treatment,the grading of GMFCS in the treatment group increased than that before treatment,and the difference was statistically significant (P<0.05),moreover,the treatment group was superior to the control group in grade V,and the difference was statistically significant (P<0.05).Before treatment,there were no statistically significant differences in DQ scores of social adaptation,individual-society,major sports,fine sports and language between the two groups (P>0.05),and they were comparable.After treatment,the abovementioned scores of the two groups all increased compared with the same group before treatment (P < 0.05 or P <0.01),moreover,the increase extent in the treatment group were superior to that in the control group,and the difference was statistically significant (P<0.01).Conclusion CE can promote the improvement of motor and cognitive function of children with dyskinetic cerebral palsy,which lays a foundation for the family rehabilitation of children through the whole participation of parents,and it is worthy of further exploration and application in clinic.
