摘要
目的探讨腹腔镜手术治疗胆囊结石伴肝硬化的临床效果.方法选取射洪县中医院2015年1月-2018年3月收治的胆囊结石伴肝硬化患者92例,随机分为对照组与观察组,各46例.对照组患者实施传统开腹手术治疗,观察组患者实施腹腔镜手术治疗.比较两组患者术后残石率、血生化指标[丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、总胆红素(TBiL)]、临床指标[术中出血量、手术时间、住院时间],并观察两组患者并发症发生情况.结果两组患者术后残石率均为0.观察组患者ALT、AST、TBiL低于对照组,术中出血量少于对照组,手术时间、住院时间短于对照组(P<0.05).观察组患者并发症发生率低于对照组(P<0.05).结论腹腔镜手术治疗胆囊结石伴肝硬化的临床效果良好,对患者肝功能生化指标影响较小,术后恢复较快,且安全性较高.
Objective To investigate the clinical effect of laparoscopic surgery for cholecystolithiasis with liver cirrhosis.Methods A total of 92 cases of patients with cholecystolithiasis and liver cirrhosis were selected from January 2015 to March 2018 in Shehong Hospital of Traditional Chinese Medicine,which were randomly divided into control group and observation group,46 cases in each group.The control group were treated with traditional laparotomy,while the observation group were treated with laparoscopic surgery.The rate of residual stone after operation was compared between the two groups;the blood biochemical indexes including ALT,AST and TBiL were compared between the two groups;and the clinical indexes of the two groups were compared,including intraoperative bleeding volume,operation time and hospitalization time;and the occurrence of complications was observed.Results The postoperative residual stone rate was 0.The observation group of ALT, AST and TBiL were lower than control group,the intraoperative bleeding volum was less than control group,the operation time and hospitalization time were shorter than control group(P<0.05).The incidence rate of complications in the observation group was lower than control group(P<0.05).Conclusion Laparoscopic surgery for cholecystolithiasis with liver cirrhosis has good clinical effect,little influence on liver function biochemical indicators,quick recovery and high safety.
