摘要
目的评价DENALI滤器植入的中长期疗效和并发症。方法回顾性分析首都医科大学附属北京世纪坛医院2016年11月至2018年11月行DENALI滤器植入的38例患者临床资料,对其围术期相关并发症、滤器取出情况进行分析及总结。结果本组患者共38例,手术均获成功,术中发生滤器倾斜1例,无其他并发症发生,手术时间(29.74±4.24)min(范围:20~40 min);随访时间(13.4±7.0)个月(范围:1~26个月)。随访期间无新发DVT和症状性PE,无滤器导致的下腔静脉闭塞。术后25例(65.8%)患者的滤器成功取出。滤器取出时间(127.7±83.1)d(范围:22~393 d);3例取出困难,取出时间(81.6±66.7)min(范围:30~330 min)。结论DENALI滤器作为一款新型的可回收滤器,具有回收时间窗长、并发症少、取出成功率高等优势,能有效预防PE的发生。
Objective To evaluate the mid-and long-term efficacy and complications of DENALI filter implantation.Methods Thirty-eight patients who underwent DENALI filter implantation from November 2011 to November 2018 in Beijing Shijitan Hospital affiliated to Capital Medical University were retrospectively reviewed and the perioperative complications and filter removal were summarized.Results A total of 38 patients in this group had successful operation,and 1 case of filter tilt occurred during operation,and no other complications occurred.The operation time was 20 minutes to 40 minutes with an average of(29.74±4.24)minutes.The follow-up period ranged from 1 month to 26 months,with an average of(13.4±7.0)months.There was no new deep vein thrombosis and symptomatic pulmonary embolism(PE)during the follow-up period,and no inferior vena cava occlusion caused by the filter.25 cases of filter were retrieved after operation,the retrieval rate was 65.8%,the retrieval success rate was 100%,the shortest indwelling time was 22 days,the longest was 393 days,the average was(127.7±83.1)days,3 cases were difficult to retrieve,and the retrieval time was 30 minutes to 330 minutes,average(81.6±66.7)minutes.Conclusions As a new type of retrievable filter,DENALI filter has the advantages of long retrievable time window,less complications and high success rate of retrieval,which can effectively prevent the occurrence of PE.
