右美托咪定联合咪达唑仑在颅脑创伤中的效果 被引量:5

The Effect of Dexmedetomidine Combined With Midazolam in Traumatic Brain Injury

摘要 目的观察颅脑创伤患者应用右美托咪定联合咪达唑仑的镇静效果及对呼吸循环功能、神经内分泌的影响。方法选取我院2016年7月-2018年7月神经外科诊治的108例颅脑创伤患者作为研究对象,将其分为两组,每组各54例。对照组镇静行右美托咪定,观察组镇静行右美托咪定+咪达唑仑,比较两组镇静评分及呼吸循环功能、神经内分泌指标。结果观察组给药后10 min、30 min、60 min镇静评分均高于对照组,差异均具有统计学意义(P<0.05);观察组镇静后HR、RR、MAP相比对照组更高,差异均具有统计学意义(P<0.01)。结论颅脑创伤患者联合应用右美托咪定、咪达唑仑,能达到良好的镇静效果,并减少呼吸循环抑制,控制神经内分泌紊乱,减轻创伤引起的应激反应,安全有效。 Objective To observe the sedative effect of dexmedetomidine combined with midazolam in patients with traumatic brain injury and its effects on respiratory and circulatory function and neuroendocrine.Methods 108 patients with craniocerebral trauma treated by neurosurgery in our hospital from July 2016 to July 2018 were selected as the subjects of study.They were divided into two groups,54 cases in each group.The control group was sedated with dexmedetomidine.The observation group was sedated with dexmedetomidine and midazolam.The sedation scores,respiratory and circulatory functions and neuroendocrine indexes were compared between the two groups.Results The sedation scores of the observation group were higher than those of the control group at 10 min,30 min and 60 min after administration,the difference was statistically significant(P<0.05).The HR,RR and MAP of the observation group were higher than those of the control group after sedation,the difference was statistically significant(P<0.01).Conclusion The combination of dexmedetomidine and midazolam in patients with traumatic brain injury can achieve good sedative effect,reduce respiratory cycle inhibition,control neuroendocrine disorders,and reduce the stress response caused by trauma.It is safe and effective.
