摘要
目的探讨伤科接骨片联合双氯芬酸钠注射液治疗闭合性足踝部骨折早期肿胀的临床疗效。方法选取2016年12月-2017年12月天全县中医医院收治的100例闭合性足踝部骨折患者,按随机数字法分为对照组和治疗组,每组各50例。对照组患者肌肉注射双氯芬酸钠注射液,50 mg/次,2~3次/d。治疗组在对照组治疗的基础上以温开水或温黄酒送服伤科接骨片,4片/次,3次/d。两组患者均连续治疗4d。观察两组的临床疗效,比较两组的临床症状改善情况和视觉模拟评分法(VAS)评分。结果治疗后,对照组和治疗组的总有效率分别为82.00%、90.00%,两组比较差异具有统计学意义(P<0.05)。治疗后,治疗组踝关节背伸、踝关节屈曲均明显大于对照组,消肿止痛时间、住院时间均明显短于对照组,两组比较差异具有统计学意义(P<0.05)。治疗后,两组患者VAS评分均显著降低,同组治疗前后比较差异具有统计学意义(P<0.05);且治疗后治疗组患者VAS评分均明显低于对照组,两组比较差异具有统计学意义(P<0.05)。结论伤科接骨片联合双氯芬酸钠注射液治疗闭合性足踝部骨折早期肿胀具有较好的临床疗效,可改善临床症状,缓解疼痛,具有一定的临床推广应用价值。
Objective To investigate the clinical effect of Shangke Jiegu Tablets combined with Diclofenac Sodium Injection in treatment of early swelling of closed ankle and foot fractures. Methods Patients(100 cases) with early swelling of closed ankle and foot fractures in Tianquan County Hospital of Traditional Chinese Medicine from December 2016 to December 2017 were randomly divided into control and treatment groups, and each group had 50 cases. Patients in the control group were intramuscular injection administered with Diclofenac Sodium Injection, 50 mg/time, 2 - 3 times daily. Patients in the treatment group were po administered with Shangke Jiegu Tablets on the basis of the control group, 4 tablets/time, three times daily. Patients in two groups were treated for 4 d. After treatment, the clinical efficacies were evaluated, and the improvement of clinical symptoms, and VAS scores in two groups were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 82.00% and 90.00%, respectively, and there was difference between two groups(P < 0.05). After treatment, the degrees of ankle dorsal extension and ankle flexion in the treatment group were bigger than those in the control group, and the times of detumescence and pain relief, and hospital stay in the treatment group were shorter than those in the control group, and there was difference between two groups(P < 0.05). After treatment, VAS scores in two groups were significantly decreased, and the difference was statistically significant in the same group(P < 0.05). And the VAS scores in the treatment group were significantly lower than those in the control group, with significant difference between two groups(P < 0.05). Conclusion Shangke Jiegu Tablets combined with Diclofenac Sodium Injection has clinical curative effect in treatment of early swelling of closed ankle and foot fractures, can improve clinical symptoms, and relieve pain, which has a certain clinical application value.
