摘要
【目的】探讨2型糖尿病肾病患者血清胆红素(TB)及血尿酸(UA)水平的变化及其意义。【方法】于本院诊治的2型糖尿病患者,其中30例尿白蛋白排泄率(UAER)正常(UAER<30 mg/24 h)的为正常组,30例2型糖尿病肾病(DN)早期(UAER 30-300 mg/24 h)患者为早期组、30例DN患者(UAER>300 mg/24 h)为DN组;比较三组糖尿病病情、肾功能指标,血清TB以及UA水平及与肾功能指标的相关性。【结果】DN组HbA1C、空腹C肽均显著高于,糖尿病病程短于正常组和早期组(P <0.05);早期组HbA1c、空腹C肽显著高于,糖尿病病程短于正常组(P <0.05);DN组UAER、血清肌酐(SCr)均显著高于,内生肌肝清除率(CCr)显著低于正常组和早期组(P <0.05),早期组UAER、SCr显著高于,CCr显著低于正常组(P <0.05);DN组UA显著高于,TB显著低于正常组和早期组(P <0.05),早期组UA显著高于,TB显著低于正常组(P <0.05);血清 TB 与 UAER、SCr、CCr 呈负相关(P <0.05),UA 水平与 UAER、SCr、CCr 呈正相关(P <0.05)。【结论】DN发生发展过程伴随着血清TB降低以及UA水平升高,其可用于DN早期辅助诊断及可能成为其临床治疗的新靶点。
[Objective] To investigate the changes and significance of serum bilirubin (TB) and uric acid (UA) levels in patients with type 2 diabetic nephropathy.[Methods] Among the type 2 diabetic patients diagnosed and treated in our hospital, 30 patients with normal urinary albumin excretion rate (UAER V 30mg/ 24h) were in the normal group, 30 patients with early type 2 diabetic nephropathy (DN)(30.300 mg/24h) were in the early group, and 30 patients with DN (>300 mg/24h) were in the DN group.The condition of diabetes mellitus, renal function index, serum TB and UA levels and their correlation with renal function index were compared among the three groups.[Results]HbAlc and fasting C-peptide in the DN group were significantly higher than those in the normal group and the early group ( P <0.05);HbAlc and fasting C-peptide in the early group were significantly higher than those in the normal group ( P <0.05);UAER and serum creatinine (SCr) in the DN group were significantly higher than those in the normal group and the early group ( P <0.05);and endogenous creatinine clearance rate (CCr) in the early group was significantly higher than those in the normal group and the early group ( P <0.05). UA in the DN group was significantly higher than that in the normal group ( P <0.05), TB in the DN group was significantly lower than that in the normal group and the early group ( P<0.05), UA in the early group was significantly higher than that in the normal group ( P <0.05), TB in serum was negatively correlated with UAER, SCr and CCr ( P <0.05), UA level was positively correlated with UAER, SCr and CCr ( P <0.05).[Conclusion]The occurrence and development of type 2 diabetic nephropathy is accompanied by the decrease of serum TB and the increase of UA level, which can be used as a new target for early diagnosis and clinical treatment of DN.
