摘要
目的:研究足细胞B7-1、尿nephrin在糖尿病肾病患者不同分期中的表达及其在预后评估中的价值。方法:选择笔者所在医院2016年2月-2018年2月治疗的糖尿病肾病患者50例,根据分期分为糖尿病肾病Ⅲ期(22例)、Ⅳ期(28例),选取糖尿病无合并肾损伤患者30例,作为糖尿病对照组,并抽取医院体检科健康体检者30例作为正常对照组,检测四组患者尿足细胞B7-1、尿nephrin的表达水平,同时观察糖尿病肾病尿足细胞B7-1、尿nephrin不同水平患者半年后eGFR的变化。结果:糖尿病肾病Ⅲ期组、糖尿病肾病Ⅳ期组尿足细胞B7-1、尿nephrin水平均高于糖尿病对照组及正常对照组,差异均有统计学意义(P<0.05)。糖尿病肾病Ⅳ期组尿足细胞B7-1、尿nephrin水平高于糖尿病肾病Ⅲ期组,差异有统计学意义(P<0.05)。糖尿病对照组及正常对照组尿足细胞B7-1、尿nephrin水平差异无统计学意义(P>0.05)。糖尿病肾病组尿B7-1、尿nephrin高表达组eGFR降低幅度均大于糖尿病肾病组尿B7-1、尿nephrin低表达组,差异均有统计学意义(P<0.05)。结论:足细胞B7-1、尿nephrin可作为糖尿病肾病患者早期诊断的重要指标,同时对糖尿病肾病患者的预后评估具有重要价值。
Objective:To study the expression of podocyte B7-1 in different stages of diabetic nephropathy and its value in prognosis evaluation. Method:A total of 50 patients with diabetic nephropathy treated in our hospital from February 2016 to February 2018 were divided into stage Ⅲ(22 cases) and stage Ⅳ(28 cases).30 patients with diabetes mellitus without renal injury were selected as diabetic control group.30 healthy persons from physical examination department of our hospital were selected as normal control group.The expression levels of urinary podocyte B7-1 and urine nephrin in four groups were detected, and the changes of eGFR in diabetic nephropathy patients with different levels of urinary podocyte B7-1 and urinary nephrin were observed half a year later. Result:The levels of urinary podocyte B7-1 and urinary nephrin in diabetic nephropathy stage Ⅲ group and diabetic nephropathy stage Ⅳ group were significantly higher than those in diabetic control group and normal control group(P<0.05).The levels of urinary podocyte B7-1 and urinary nephrin in stage Ⅳ diabetic nephropathy group were significantly higher than those in diabetic nephropathy stage Ⅲ group(P<0.05).There was no significant difference in urinary podocyte B7-1 and urine nephrin levels between diabetic control group and normal control group(P>0.05).The decrease of eGFR in the urine of the diabetic nephropathy group was greater than that in the diabetic nephropathy group,and the urine nephrin low expression group was significantly higher than that of the diabetic nephropathy group(P<0.05).Conclusion:Podocyte B7-1 and urinary nephrin can be used as important indicators for early diagnosis of diabetic nephropathy,and have important value for prognosis evaluation of diabetic nephropathy.
