摘要
[目的]观察乌梅丸联合匹维溴铵治疗腹泻型肠易激综合征(寒热错杂)疗效。[方法]使用随机平行对照方法,将46例门诊患者病按就诊顺序号随机分为两组。对照组23例匹维溴铵,4mg/次,2次/d。治疗组23例乌梅丸(桂枝10g,乌梅、黄柏、炮姜各15g,制附子6g,细辛3g,党参15g,当归9g,花椒6g,黄连10g),水煎400mL,1剂/d,2次/d。腹痛严重、少腹拘急、肝郁乘脾加炙甘草10g,白芍15g;脾虚湿重、黏液便加生苡仁15g,苍术10g,白扁豆15g;瘀血阻滞和舌质黯加川芎6g;脾虚夹热、舌苔黄黄连增至15g;阳虚、形寒肢冷制附片用量增至15g;食欲不振和纳呆,神曲、焦楂各15g;西药治疗同对照组。连续治疗3周为1疗程。观测临床表现、中医证候积分、不良反应。连续治疗2疗程(6周),判定疗效。[结果]治疗组治愈5例,显效11例,有效4例,无效3例,总有效率86.96%;对照组治愈3例,显效8例,有效4例,无效8例,总有效率65.22%;治疗组疗效优于对照组(P<0.05)。中医证候积分两组均有改善(P<0.01),治疗组改善优于对照组(P<0.01)。[结论]乌梅丸联合匹维溴铵治疗腹泻型肠易激综合征(寒热错杂),疗效满意,无严重不良反应,值得推广。
[Objective] To observe the curative effect of Wumei pill combined with pivobromide on diarrheapredominant irritable bowel syndrome.[Method] 46 outpatients were randomly divided into two groups according to the order number of visits by randomized parallel control method. The control group consisted of 23 cases of pivobromide, 4 mg/time, 2 times/day. In the treatment group, 23 cases of black plum(Guizhi10 g, Wumei, Huangbai, Paojiang each 15 g, Zhifuzi 6 g, Xixin 3 g, Dangshen 15 g, Danggui 9 g, Huajiao6 g, Huanglian 10 g) were decocted with water 400 mL, 1 dose/d, twice a day. Severe abdominal pain, less abdominal constraints, liver depression, spleen plus liquorice roasted 10 g, Paeonia lactiflora 15 g;spleen deficiency and wet weight, mucus plus raw seeds 15 g, Atractylodes Rhizoma 10 g, lentil 15 g;blood stasis block and tongue dim plus Ligusticum chuanxiong 6 g;spleen deficiency and heat, tongue coating Coptis chinensis increased to 15 g;Yang deficiency, cold limb attached tablet dosage increased to 15 g;appetite inactivation and dullness, Shenqu, Jiaohu each 15 g;western medicine treatment with the same control group. Continuous treatment for 3 weeks is a course of treatment. Observation of clinical manifestations, TCM syndrome scores,adverse reactions. Continuous treatment for 2 courses(6 weeks) to determine the efficacy.[Result] In the treatment group, 5 cases were cured, 11 cases were markedly effective, 4 cases were effective and 3 cases were ineffective, the total effective rate was 86.96%. In the control group, 3 cases were cured, 8 cases were markedly effective, 4 cases were effective and 8 cases were ineffective, the total effective rate was 65.22%.The curative effect of the treatment group was better than that of the control group(P<0.05). The scores of TCM syndromes were improved in both groups(P<0.01), and the improvement in the treatment group was better than that in the control group(P<0.01).[Conclusion] Wumei pill combined with pivobromide in the treatment of diarrhea-predominant irritable bowel syndrome(Hanre Cuoza) has satisfactory curative effect and no serious adverse reactions, which is worthy of promotion.
