摘要
目的:分析讨论对特发性间质性肺炎患者使用环磷酰胺和泼尼松联合治疗的具体效果和实际价值。方法:选取本院2015年1月-2017年12月特发性间质性肺炎(idiopathic interstitial pneumonia,IIP)患者76例,根据入院先后将其均分为两组,每组各38例。对照组给予泼尼松进行治疗,观察组在服用泼尼松的基础上继续服用环磷酰胺。比较两组治疗前后呼吸困难修正评分量表(MMRC)评分、ADL生活质量评分、肺功能和总有效率。结果:治疗前,两组MMRC评分和ADL评分比较,差异均无统计学意义(P>0.05);治疗后,观察组MMRC评分低于对照组,ADL生活质量评分高于对照组(P<0.05)。治疗前,两组各项肺功能指标比较,差异均无统计学意义(P>0.05);治疗后,观察组各项肺功能指标均明显优于对照组(P<0.05)。观察组总有效率94.74%明显高于对照组71.10%,差异有统计学意义(P<0.05)。结论:对特发性间质性肺炎患者使用环磷酰胺和泼尼松联合治疗能提高患者生活质量,降低呼吸难度,并改善肺部功能,整体治疗效果优异,可在临床上进行全面推广使用。
Objective:To analyze and discuss the specific effect and practical value of Cyclophosphamide plus Prednisone in the pulmonary function for idiopathic interstitial pneumonia(IIP)patients.Method:76 IIP patients treated from January 2015 to December 2017 in our hospital were selected and divided into control group(n=38)and observation group(n=38)according to the admission sequence,38 cases in each group.The control grouptreatedwithPrednisone.While the observation group treated with Cyclophosphamide on the basis of Prednisone.The modified British medical research council(MMRC)scores,activity of daily living(ADL)scores,pulmonary function and total effective rate between the two groups before and after treatmentwerecompared.Result:Before treatment,the MMRC score and ADL score of the two groups were compared,the differences were not statistically significant(P>0.05).After treatment,the MMRC score in the observation group was lower than that in the control group,while the ADL score in the observation group was higher than that in the control group(P<0.05).Before treatment,the pulmonary function of the two groups were compared,the differences were not statistically significant(P>0.05).After treatment,the pulmonary function in the observation group was significantly better than that in the control group(P<0.05).The total effective rate in the observation group was 94.74%significantly higher than the control group of 71.10%,the difference was no statistically significant(P<0.05).Conclusion:The combined therapy of Cyclophosphamide and Prednisone for patients with IIP can improve the life quality,relieve the dyspnea,improve the pulmonary function.The overall treatment effect is excellent,it can be widely used in clinical practice.
