摘要
目的:探讨多层螺旋CT在肥胖合并阻塞性睡眠呼吸暂停综合征(OSAHS)检查中的应用。方法:选取2017年1月至2018年10月甘肃省民勤县人民医院收治的肥胖合并OSAHS患者30例为观察组,选择同期30例肥胖非OSAHS患者为对照组。回顾性分析2组的相关资料,所有患者均接受多层螺旋CT检查及三维重建。比较2组的检查结果。结果:在软组织测量指标上,2组软腭面积、体积比较,差异有统计学意义(P<0.05);而2组舌面积、体积等比较,差异无统计学意义(P>0.05)。在骨性结构测量指标上,2组的舌骨升支体积、舌骨体体积、硬腭长度、下颌骨体积、截面积比较,差异无统计学意义(P>0.05)。在空间结构测量上,2组口咽部气道体积、前后径、截面积比较,差异有统计学意义(P<0.05);而2组的口咽部气道左右径比较,差异无统计学意义(P>0.05)。肥胖合并OSAHS患者的软腭体积与BMI、AHI呈明显相关性(未进行相关性分析,建议删除或者补充相关性分析)。结论:肥胖合并OSAHS患者采用多层螺旋CT检查可准确、客观的描述其疾病症状,而疾病的严重程度与软腭体积增大具有一定关系。
Objective:To explore the application of multi-slice spiral CT in(OSAHS)examination of obese patients with obstructive sleep apnea syndrome(OSAS).Methods:A total of 30 obese patients with OSAHS from January 2017 to October 2018 were selected as observation group and 30 obese patients without OSAHS were selected as control group.The data of all patients were retrospectively analyzed,and they were all detected with multi-slice spiral CT and three-dimensional reconstruction.The results of two groups were compared.Results:The soft tissue measurement indexes including soft palate area and volume of two groups were compared,the differences were all statistically significant(P<0.05);the tongue area and volume of two groups were compared,the differences were not statistically significant(P>0.05).The bony structure measurement indexes including volume of ascending branch of hyoid,the volume of hyoid body,the length of hard palate,the volume of mandible and the cross section area of two groups were compared,the differences were all statistically significant(P<0.05).The space structure measurement indexes including airway volume,anterior and posterior diameter and cross section area of two groups were compared,the differences were all statistically significant(P<0.05);while the left and right airway diameters of two groups were compared,the differences were not statistically significant(P>0.05).Conclusion:The multi-slice spiral CT can be used to describe the symptoms of obesity patients with OSAHS accurately and objectively,and the severity of the disease is related to the enlargement of soft palate.
