摘要
目的观察黄斑OCT各参数在评估糖尿病性黄斑水肿(diabetic macular edema,DME)患者在接受抗VEGF治疗后效果中的应用价值。方法选取DME患者86例108只眼,其中男性51例70眼,女性35例38眼。在抗VEGF(vascular endothelial growth factor,VEGF)治疗前后进行黄斑OCT及BCVA(best corrected visual acuity,BCVA)检查。记录黄斑中心凹高度、黄斑区1mm容积、黄斑区3mm容积、黄斑区6mm容积共4个OCT参数。将4个参数变化与BCVA变化进行Pearson相关性分析及ROC分析。结果经过Pearson相关性分析,OCT各参数均与BCVA的变化明显相关(黄斑中心凹厚度r=-0.277,P=0.006;黄斑区1mm容积r=-0.257,P=0.012;黄斑3mm容积r=-0.204,P=0.049;黄斑6mm容积r=-0.212,P=0.038)。经绘制ROC曲线发现,黄斑OCT各参数与BCVA均有较好的符合性。黄斑中心凹高度的曲线下面积为0.672(P=0.007),黄斑区1mm体积曲线下面积0.670(P=0.008),黄斑区3mm体积曲线下面积0.655(P=0.015),黄斑区6mm体积曲线下面积0.637(P=0.032)。结论黄斑OCT各参数在对糖尿病性黄斑水肿疗效均有很好的评估价值,这4个参数的评估作用是类似的。
Objective To observe the value of parameters of optic coherence tomography in evaluating diabetic macular edema after anti-VEGF treatment.Methods Eighty-six subjects(108 eyes) with diabetic macular edema who got anti-VEGF treatment were enrolled in this study.The macular OCT and the best corrected visual acuity were obtained and the thickness of central fovea of macular,volume of macular in 1mm,3mm and 6mm of OCT were measured.The receiver operating characteristic (ROC) curve was made to evaluated the ability of OCT parameters in detect progression of DME after anti-VEGF treatment.Results All the OCT parameters were significantly correlated with BCVA (the thickness of central fovea of macular r =-0.277,P =0.006;volume of macular in 1mm r =-0.257,P =0.012;volume of macular in 3mm r =-0.204,P =0.049;volume of macular in 6mm r =-0.212,P =0.038).The AROC of thickness of central fovea was 0.672( P =0.007),volume of 1mm was 0.670( P =0.008),volume was 0.655( P =0.015),volume of 6mm was 0.637 ( P =0.032).Conclusion All the parameters of OCT have equal and good evaluating ability in detect progression of DME after anti-VEGF treatment.
