摘要
目的探讨脱钙人牙基质材料混合自体髂骨打压植骨在早期股骨头坏死保髋治疗中的应用效果。方法选择2015年7月—2017年6月该院收治的46例早期股骨头坏死患者作为研究对象,以随机数字表将其分为对照组与研究组,每组各23例。对照组采用髓芯减压与自体髂骨打压植骨治疗,研究组在此基础上联用脱钙人牙基质材料。结果术后12个月时,研究组JOA(24.5±1.5)分与Harris评分(85.6±6.3)分均高于对照组(20.5±2.0)分、(78.5±6.2)分与术前(12.5±2.2)分、(66.5±7.6)分[与对照组术后12个月对比(t=3.658,4.052,P<0.05)];与术前对比,t=6.523,6.856,P<0.05)]。对照组术后出现塌陷2例(8.70%),研究组未见塌陷,对比差异无统计学意义(χ^2=0.523,P>0.05)。结论脱钙人牙基质材料混合自体髂骨打压植骨在早期股骨头坏死保髋治疗具有显著的应用效果。
Objective To investigate the effect of decalcified human dental matrix material combined with autologous iliac bone compression and bone grafting in the treatment of early avascular necrosis of femoral head.Methods 46 cases of early femoral head necrosis in our hospital from July 2015 to June 2017 were selected and randomly divided into control group and research group with 23 cases in each group.The control group was treated with core decompression and autologous iliac bone compression and bone grafting,and the study group was treated with decalcified human dental matrix material on this basis.Results At 12 months after surgery,the JOA (24.5±1.5)points and the Harris score (85.6±6.3)points in the study group were higher than those in the control group (20.5±2.0),(78.5±6.2),and preoperative (12.5±2.2)points,(66.5±7.6) points [compared with the control group 12 months after surgery(t=3.658,4.052,P<0.05);compared with preoperative(t=6.523,6.856,P<0.05)].There were 2 cases (8.70%) of collapse in the control group,and there was no collapse in the study group.The difference was not statistically significant (χ^2=0.523,P>0.05).Conclusion Decalcified human dental matrix material combined with autologous iliac bone compression and bone grafting has significant application effect in hip-conserving treatment of early femoral head necrosis.
