摘要
目的:应用六西格玛理论对血常规检验项目的质量水平进行评价,为质量改进提供科学依据。方法:收集2017年希森美康XN-1000血细胞分析仪各检验项目的室内质量控制变异系数(CV)及国家卫计委临检中心室间质量评价偏倚(Bias),根据各项目的CV、Bias及总允许误差(TEa)计算其西格玛值(σ)和质量目标指数(QGI),评价各检验项目分析性能,查找性能不佳的主要原因。结果:8项血常规检验项目均达到3σ水平,σ水平从高到低依次是 MCH(7.29σ)、Pit(5.83σ)、RBC (4.92σ)、Het(4.66σ), WBC (4.51σ), MCV(4.48σ)、Hb (4.34σ)及MCHC (3.97σ)。目前需要优先改进精密度的项目有WBC、RBC、Hb及Plt,需优先改进正确度的是Hct及MCV,而需同时改进精密度和正确度的是MCHC。结论:六西格玛理论对临床实验室血常规检验项目的质控方案选择及质量改进具有重要价值,是一种简便、实用的质量管理工具。
Objective: To evaluate the quality level of the blood routine inspection project and provide a scientific basis for the improvement of quality using six sigma theory. Methods: The coefficient of variation of indoor quality control ang the bias of inter-chamber quality evaluation of National Health and Welfare Commission clinical examination center on hematology analyzer Sysmex XN-1000 were collected in 2017. The sigma value and quality target index were calculated according to coefficient of variation, bias and the allowable error, and the analysis perfonnance of each inspection project was evaluated, and then the main reasons for poor performance were found out. Results: All of the eight items in blood routine test reached three sigma level. The sigma levels of MCH, Pit, RBC, Het, WBC, MCV, Hb and MCHC were 7.29, 5.83, 4.92, 4.66, 4.51, 4.48, 4.34 and 3.97 respect from high to low. The precision of WBC, RBC, Hb and Pit was needed to improve preferentially, the accuracy of Het and MCV was needed to improve preferentially, and the precision and accuracy of MCHC were needed to improve preferentially at the same time. Con clusi Ofl: Six Sigma theory is of great value to quality control program selection and quality improvement of clinical laboratory blood test items. It is a simple and practical quality management tool.
