摘要
目的探讨肝癌病理诊断中肝组织雌激素受体α表达的临床价值。方法选取本院肝胆外科2014年1月至2016年1月手术切除并经病理证实为肝细胞癌患者70例,采用免疫组化法检测患者癌组织及癌旁组织中雌激素受体α(ERα)的表达情况,分析ERα表达与患者临床病理特征的关系及与患者术后无瘤生存率、总生存率的关系。结果 ERα在肝癌组织中阳性表达率为31.43%,癌旁组织ERα的阳性表达率为62.50%,差异有统计学意义(P<0.05),肝癌组织表达的ERα的解离常数显著高于癌旁组织(P<0.05);肿瘤大小≤5 cm、血清甲胎蛋白水平<400μg/L、包膜完整及无镜下癌栓患者ERα表达阳性率显著高于肿瘤大小>5 cm、血清甲胎蛋白水平>400μg/L、无完整包膜及有镜下癌栓患者,且术后随访期间ERα表达阳性患者总体生存率与无瘤生存率显著高于ERα表达阴性患者(P<0.05)。结论在肝癌组织中ERα表达阳性率显著低于癌旁组织,且ERα表达与肝癌患者肿瘤生物学特性及术后的预后有一定关系。
Objective To investigate the clinical value of the expression of estrogen receptor α(ERα) in liver tissue in the pathological diagnosis of liver cancer.Methods Seventy patients with pathologically confirmed hepatocellular carcinoma who underwent surgical resection in hepatological surgery department of the hospital during the period from January 2014 to January 2016 were enrolled in the study.The expression of ERα in cancer tissues and adjacent tissues was detected by immunohistochemical method.The relationships of the expression of ERα with clinicopathological features,postoperative tumor-free survival rate and overall survival rate were analyzed.Results The positive expression rates of ERα in liver cancer tissues and adjacent tissues were 31.43% and 62.50% respectively (P< 0.05).The dissociation constant of ERα expressed in liver cancer tissues was significantly higher than that in adjacent tissues (P<0.05).The positive expression rate of ERα was significantly higher in patients with tumor≤5 cm, serum alpha-fetoprotein level<400 μg/L,intact capsule and without microscopic cancer embolus significantly than in patients with tumor>5 cm,serum alpha-fetoprotein level>400 μg/L,without complete capsule and with microscopic tumor embolus.Besides,the overall survival rate and tumorfree survival rate of patients with positive expression of ERa during postoperative follow-up were significantly higher than those with negative expression of ERa (P<0.05).Conclusion The expression positive rate of ERa in liver cancer tissues is significantly lower than that in adjacent tissues, and the expression of ERa is related to biological characteristics of tumors and the prognosis of patients with liver cancer.
