摘要
目的分析PG、G-17及Hp-IgG抗体检测对早期胃癌的筛查效果。方法选择2018年1—10月我院确诊的30例早期胃癌患者(观察组)、34例非萎缩性胃炎患者(对照1组)及30例同期行健康体检者(对照2组)作为研究对象。分析上述受试者的血清胃蛋白酶原(PG)、胃泌素-17(G-17)及幽门螺旋杆菌IgG(Hp-IgG)抗体结果。结果 PGⅠ、PGR与G-17按照观察组、对照组1组、对照2组的次序逐渐升高,差异有统计学意义(P <0.05)。观察组和对照1组的PGⅡ高于对照2组,差异有统计学意义(P <0.05)。联合诊断的灵敏度(33.3%~73.3%)较单项诊断的灵敏度(76.6%~83.3%)低,而特异度(70.2%~96.9%)较单项诊断的特异度(56.3%~67.2%)高。诊断准确率比较中,PG阳性与G-17阳性联合诊断胃癌准确率最高(与仅次于该联合的Hp-IgG抗体、PG及G-17三者联合相比,χ~2=4.447 3,P=0.035 0)。结论单项检测进行胃癌筛查,则PG检测为首选,联合检测进行胃癌筛查则以PG联合G-17检测作为首选。
Objective To analyze the screening effect of serum pepsinogen,gastrin-17 and Helicobacter pylori IgG antibodies for early gastric cancer.Methods From January to October 2018,30 patients with early gastric cancer (observation group),34 patients with non-atrophic gastritis (control group 1) and 30 patients with health examination (control group 2) were selected as the study subjects.Serum pepsinogen (PG),gastrin-17 (G-17) and IgG antibody of Helicobacter pylori (Hp-IgG) were analyzed.Results PG I,PGR and G-17 increased gradually according to the order of observation group,control group 1 and control group 2 (P < 0.05).The PG II of the observation group and the control group 1 was higher than that of the control group 2,and the difference was statistically significant (P < 0.05).The sensitivity of combined diagnosis (33.3%~73.3%) was lower than that of single diagnosis (76.6%~83.3%),while the specificity (70.2%~96.9%) was higher than that of single diagnosis (56.3%~67.2%).Compared with the combi nation of HP-IgG antibody,PG and G-17,the combination of PG and G-17 had the highest diagnostic accuracy (χ^2 = 4.447 3,P = 0.0350).Conclusion PG detection is the first choice for single detection in gastric cancer screening,and PG combined with G-17 detection is the first choice for combined detection in gastric cancer screening.
