分析急诊外科创伤患者切口感染的危险因素 被引量:4

摘要 目的分析急诊外科创伤患者切口感染的危险因素。方法选取2017年12月—2018年12月在我院急诊外科收治的创伤患者170例作为本次实验的研究对象,以术后切口是否发生感染为依据分成对照组和病例组。对患者的性别、年龄、创伤部位、身体质量指数(BMI)、创伤分数(TS)、创伤数、抗菌药物术前使用情况、手术时间等因素进行Logistic多元回归分析和单因素分析。结果 170例患者中,27例术后发生了切口感染,感染率15.88%;在单因素分析结果中,年龄≥60岁、头部创伤、BMI> 24 kg/m^2、多处创伤、TS≤4分、术前并未应用抗菌药物是切口术后发生感染的主要诱因(P <0.05)。在多元回归分析结果中,年龄≥60岁、TS≤4分、BMI> 24 kg/m^2是诱发患者伤口出现感染的主要因素。结论在对急诊外科创伤手术患者进行处理的过程中,术前评估工作一定要做好,及时干预可能感染伤口的危险因素,将消毒、清理工作做好,可预防采用抗菌药物,减少感染创伤手术切口的几率。 Objective To analyze the risk factors of incision infection in patients with emergency surgical trauma.Methods 170 cases of trauma patients admitted to the emergency surgery department of our hospital from December 2017 to December 2018 were selected as the research objects of this experiment,and divided into control group and case group according to whether infection occurred in the incision after surgery.Logistic multiple regression analysis and univariate analysis were performed on the factors including gender,age,trauma site,body mass index (BMI),trauma score (TS),trauma number,preoperative use of antibacterial agents,and operation time.Results Among the 170 patients,27 had postoperative incision infection,with an infection rate of 15.88%.In the univariate analysis results,age ≥ 60 years old,head trauma,BMI > 24 kg/m2,multiple trauma,TS ≤ 4 scores,and the absence of antibacterial agents were the main causes of postoperative infection (P < 0.05).In the results of multiple regression analysis,age ≥ 60 years old,TS ≤ 4 points,and BMI > 24 kg/m2 were the main factors inducing wound infection in patients.Conclusion In the process of treating patients with emergency surgical trauma surgery,preoperative assessment must be done well,timely intervention of the risk factors that may infect the wound,disinfection and cleaning should be done well,and antibacterial drugs can be used for prevention,so as to reduce the probability of infected wound incision.
