摘要
目的探讨术前睑板腺处理对合并睑板腺功能障碍(Meibomian gland dysfunction,MGD)的翼状胬肉患者术后眼表的影响。方法选取患有MGD的翼状胬肉患者90眼,随机分为A组(48眼)、B组(42眼),A组术前1周予以清洁眼睑、热敷及药物治疗,并同时配合睑板腺按摩治疗,B组采取常规翼状胬肉手术术前准备。两组患者均行翼状胬肉切除联合角膜缘干细胞移植术。分别于术后3 d,术后1周、2周、4周比较两组的眼部舒适度评分及泪膜破裂时间(break-up time,BUT)。结果术后3 d、1周、2周与B组对比,A组眼部舒适度评分均低于B组(P<0.01),术后1周、2周与B组对比,A组泪膜破裂时间明显长于B组,差异有统计学意义(P<0.01)。结论术前睑板腺处理可以有效促进翼状胬肉合并MGD患者术后眼表的修复,稳定泪膜,提高患者的眼部舒适度。
Objective To investigate the effect of preoperative meibomian gland treatment on postoperative ocular surface of patients with pterygium and meibomian gland dysfunction(MGD).Methods 90 eyes with pterygium and MGD were randomly divided into group A(48 eyes)and group B(42 eyes).Group A was given cleaning eyelids,hot compresses and drug treatment at 1w before surgery,combined with meibomian gland massage treatment.Group B was given conventional preoperative preparation for pterygium surgery.Both groups underwent pterygium excision combined with limbal stem cell transplantation.The eye comfort score and tear film break-up time(BUT)of the two groups were compared at 1 week,2w,and 4w after surgery.Results Compared with that of group B,the eye comfort scores of group A were lower than those of group B at 3d,1w,2w after surgery(P<0.01).The tear film rupture time of group A was significantly longer than that of group B at 1w,2w after surgery,and the difference was statistically significant(P<0.01).Conclusion Preoperative meibomian gland treatment can effectively promote the repair of ocular surface of pterygium and MGD patients,stabilize the tear film and improve the patient's eye comfort.
