摘要
目的探讨不同穿刺点进行锁骨下静脉置管的效果。方法该研究方便选取院内2016年1月—2018年4月收治的240例锁骨下静脉穿刺置管患者作为研究对象,将其分为穿刺1组、穿刺2组与穿刺3组,穿刺1组患者的穿刺点均为锁骨中内1/3处,穿刺2组患者的穿刺点均为锁骨中外1/3处,穿刺3组患者的穿刺点均为锁骨中点下方处,观察患者穿刺成功率、穿刺时间及并发症情况。结果穿刺1组、穿刺2组、穿刺3组的穿刺成功率分别为97.50%、95.00%、88.75%。置管时间分别为:(6.62±2.78)s、(11.86±3.13)s、(11.79±3.25)s。并发症发生率分别为:6.25%、10.00%、12.50%。说明穿刺1组的成功率高于穿刺2组与穿刺3组,差异有统计学意义(P<0.05);另外,穿刺1组的并发症发生率以及操作时间均低于穿刺2组与穿刺3组,差异有统计学意义(P<0.05)。结论锁骨内侧组要优于锁骨中点组及锁骨外侧组,具有更好的锁骨下静脉置管效果,该方法宜在临床中推广。
Objective To investigate the effect of different puncture points for subclavian vein catheterization. Methods A total of 240 patients with subclavian vein catheterization admitted to the hospital from January 2016 to April 2018 were convenient selected and enrolled in the study. They were divided into puncture group 1, puncture group 2 and puncture group 3, and puncture group 1 of the puncture point is 1/3 of the clavicle. The puncture point of the puncture group is 1/3 of the clavicle. The puncture point of the puncture group is below the midpoint of the clavicle. The puncture success rate,puncture time and complications were compared. Results The puncture success rates of puncture group 1, puncture group 2 and puncture group 3 were 97.50%, 95.00% and 88.75%, respectively. The catheterization time was(6.62±2.78)s,(11.86±3.13)s, and(11.79±3.25)s, respectively. The complication rates were 6.25%, 10.00%, and 12.50%, respectively. The success rate of puncture group 1 was higher than that of puncture group 2 and puncture group 3,the different was statistically significant(P <0.05). In addition, the complication rate and operation time of puncture group 1 were lower than those of puncture group 2 and puncture group 3,the different was statistically significant(P<0.05). Conclusion The medial clavicle group is superior to the clavicular midpoint group and the lateral clavicle group, and has better subclavian vein catheterization effect. This method should be popularized in the clinic.
