摘要
目的探讨血液透析患者心血管事件发生的相关危险因素及干预对策。方法选择2017年8月至2018年5月我院收治的血液透析患者146例作为研究对象,根据是否发生心血管事件分为发生心血管事件组32例和未发生心血管事件组114例。查阅病历记录并统计患者性别、年龄、血压、血液透析时间、白蛋白(ALB)、血脂四项、尿素氮、血肌酐、胱抑素C(Cys-C)、超敏C反应蛋白、血磷、血钙、血尿酸(UA)及全段甲状旁腺素(i PTH)等因素,对上述因素进行比较并进行多因素Logistic回归分析。结果血液透析患者心血管事件发生与患者性别、年龄、舒张压、血液透析时间、血脂四项、尿素氮、血肌酐、Cys-C、血磷、血钙、ALB无相关性(P> 0. 05);血液透析患者心血管事件发生率与收缩压、UA、超敏C反应蛋白、i PTH水平具有相关性(P <0. 05)。结论血液透析患者心血管事件发生危险因素较多,应根据危险因素采取干预措施,降低心血管事件发生率。
Objective To investigate the risk factors related to cardiovascular events in hemodialysis patients and its intervention strategies. Methods To select 146 cases of hemodialysis patients who were admitted to our hospital from August 2017 to May 2018 as the study subjects. These patients were divided into cardiovascular events group with 32 cases and non-cardiovascular events group with 114 cases according to whether they had cardiovascular events. We reviewed medical records and counted factors such as patient's gender, age, blood pressure, hemodialysis time, albumin (ALB), lipid profile, urea nitrogen, serum creatinine, cystatin C (Cys-C), hypersensitive C-reactive protein, serum phosphate, serum calcium, blood uric acid (UA) and intact parathyroid hormone (iPTH). These factors were compared and analyzed by the multivariate Logistic regression analysis. Results There was no correlation between incidence of cardiovascular events in hemodialysis patients and patient's gender, age,diastolic blood pressure, hemodialysis time,blood lipids, urea nitrogen, serum creatinine, Cys-C, serum phosphate, serum calcium and ALB(P>0.05). The incidence of cardiovascular events in hemodialysis patients was correlated with systolic blood pressure, UA, hypersensitive C-reactive protein and iPTH levels(P<0.05). Conclusion There are many risk factors for cardiovascular events in hemodialysis patients. We should take interventions according to risk factors, thus reducing the incidence of cardiovascular events.
