摘要
目的观察吻合器痔上黏膜环形切除术治疗重度痔疮的临床效果及对患者肛肠动力学、疼痛应激的影响。方法将2017年2月～2018年8月我院收治的76例重度痔疮患者随机分为对照组和观察组各38例。对照组患者给予外剥内扎术,观察组则给予吻合器痔上黏膜环形切除术。比较两组手术效果,测定肛肠动力学指标,于术后不同时间点评估肛周疼痛应激程度。结果与对照组相比,观察组患者术中出血量明显减少,手术时间、创面愈合时间明显缩短(P<0.05)。术后1个月,观察组患者的肛肠动力恢复优于对照组,HPZ、MSP、MTV均较对照组显著升高,而RRP、AIRT则较对照组显著降低(P<0.05);术后观察组患者不同时间段的疼痛应激VAS评分均明显低于对照组(P<0.05)。结论 PPH术创伤小、患者术后恢复快,且对患者肛肠动力学影响小,术后疼痛应激小。
Objective To observe the clinical effect of procedure for prolapse and hemorrhoids on severe hemorrhoids and its influence on anorectal dynamics and pain stress in patients. Methods 116 patients with severe hemorrhoids admitted in our hospital from February 2017 to August 2018 were randomly divided into control group and observation group, with 58 cases in each group. The patients in the control group were treated with external stripping and internal ligation, and the observation group was given procedure for prolapse and hemorrhoids. The effects of the two groups were compared. The anorectal kinetics index was measured, and the degree of pain in the perianal pain was evaluated at different time points after surgery. Results Compared with that of the control group, the intraoperative blood loss was significantly reduced in the observation group, and the operation time and wound healing time in the observation group were significantly shortened(P<0.05). At 1 month after operation, the anorectal motility recovery of the observation group was better than that of the control group. HPZ, MSP and MTV in the observation group were significantly higher than those of the control group, while RRP and AIRT in the observation group were significantly lower than those of the control group(P<0.05). The VAS scores of pain stress in the observation group after surgery were significantly lower than those in the control group(P<0.05). Conclusion PPH is less traumatic, and the patient recovers quickly after surgery. It has little effect on the anorectal dynamics of patients and has little postoperative pain stress.
