摘要
目的:观察大承气汤鼻饲对ICU患者(肠胃燥热型)胃肠功能的恢复作用。方法:选择ICU收治的危重病患者100例,分为观察组和对照组,每组50例。对照组在胃肠道护理中给予西药常规护理,观察组患者在对照组的基础上,给予大承气汤鼻饲,连续治疗7天为1个疗程。比较两组治疗前后胃肠道功能评分及APACHE-Ⅱ评分、DAO、IL-6和CRP水平,胃肠功能改善情况、胃肠道并发症发生情况。结果:观察组胃肠道功能评分及APACHE-Ⅱ评分显著低于对照组,观察组患者腹胀缓解、肠鸣音恢复、肛门首次排气以及排便时间显著低于对照组,观察组肠麻痹和胃肠功能衰竭发生率显著低于对照组,观察组治疗后DAO、IL-6和CRP水平显著低于对照组,P<0.05,差异有统计学意义。结论:大承气汤鼻饲可以改善胃肠道功能,降低胃肠道并发症发生率,减少炎性介质的释放,改善预后。
Objective: To evaluate the effect of Dachengqi Decoction on the recovery of gastrointestinal function for ICU patients. Methods: 100 critically ill patients treated with ICU were selected and divided into observation group and controlled group,with 50 in each group. The controlled group was given routine western medicine nursing in gastrointestinal nursing. The observation group was given Dachengqi Decoction nasal feeding on the basis of the controlled group,and continuous treatment for 7 d was a course of treatment. Gastrointestinal function score and APACHE-II score,DAO,IL-6 and CRP levels,gastrointestinal function improvement and gastrointestinal complications were compared between the two groups before and after treatment. Results: The gastrointestinal function score and APACHE-score of the observation group of were significantly lower than the controlled group,the abdominal distension,bowel sounds recovery,anal exhaust and defecation time of the observation group was significantly lower than the controlled group,the observation group of paralytic ileus and gastrointestinal function failure were significantly lower than the controlled group,the observation group after treatment,DAO,IL-6 and CRP levels significantly lower than the controlled group,P<0.05,the difference was statistically significant. Conclusion: Dachengqi Decoction can improve the function of gastrointestinal tract,reduce the incidence of gastrointestinal complications,reduce the release of inflammatory mediators and improve the prognosis.
