摘要
目的探讨血清PCT,IL-6与CRP水平检测对白血病患者化疗并发败血症的诊断价值。方法选取2013年1月~2018年7月进行白血病化疗患者252例,包括感染期组和恢复期组(152例),未感染组(100例);检测各组患者血清PCT,IL-6与CRP水平,并比较组间的差异;分析三项指标间及其与白血病患者化疗并发败血症的相关性;分析三项指标及其联合检测在诊断白血病患者化疗并发败血症的价值。结果感染期组PCT,IL-6与CRP水平均高于恢复期组及未感染组,差异有统计学意义(t=6.119~15.078,均P=0.000);三项指标互为相关(r=0.298~0.503,均P=0.00),并均与白血病化疗并发败血症有显著相关性(wald=4.010~21.989,P=0.000~0.045);PCT+CRP联合检测与PCT+IL-6+CRP联合检测的最高AUC分别为0.970和0.972,两曲线经Z检验差异无统计学意义(z=0.157,P=0.875)。结论 PCT,CRP和IL-6三项指标在白血病患者化疗并发败血症的诊断中有良好的应用价值。
Objective To explore the diagnostic value of serum PCT,IL-6and CRP in septicemia patients with leukemia in chemotherapy.Methods 252cases of patients with leukemia in chemotherapy were selected from January 2013to July 2018,which were divided into infection group,recovery group and uninfected group.The serum PCT,IL-6and CRP levels of each group were detected,and the differences of each groups were compared.The correlation among the three indicators and the correlation between each indicators and septicemia with Leukemia in chemotherapy were analyzed.The diagnosis value of each indicators and their combination in septicemia patients with leukemia in chemotherapy were analyzed by regression analysis and the ROC curve.Results The levels of PCT,IL-6and CRP in infection group were higher than in recovery group and uninfected group,and the difference was statistically significant(t=6.119~15.07,all P=0.000).Three indicators were related with each other(r=0.298~0.503,all P=0.000),which all were significantly correlated with septicemia with Leukemia in chemotherapy(wald=4.010~21.989,P=0.000~0.045).The combination of PCT+CRP and PCT+IL-6+CRP had highest AUC:0.970and 0.972,but the two curves were no statistically significant difference by Ztest(z=0.157,P=0.875).Conclusion PCT,CRP and IL-6indicators have a good diagnostic value in Septicemia patients with leukemia in chemotherapy.
