摘要
1例65岁女性患者,首日静脉给予喜炎平注射液联合注射用阿奇霉素治疗,未见异常;第2次给予喜炎平注射液1 h后,头面部、颈部出现散在皮疹,无瘙痒感,体温38℃,排除食物及其他药物影响,立即停用喜炎平注射液;后深红色皮疹较前增多,胸部、背部及四肢有新发皮疹,予以盐酸苯海拉明注射液抗过敏治疗;皮疹较前继续扩大,并融合呈片状,体温39.7℃,考虑为多形红斑,给予注射用甲泼尼龙琥珀酸钠、葡萄糖酸钙注射液、依巴斯汀片及富马酸酮替芬片,并行物理降温;经过治疗,症状好转。该不良反应很可能是由喜炎平注射液引起。中药注射剂成分复杂,含有多种致过敏成分,老年患者尤其是高敏体质者需谨慎使用。
One 65-year-old female patient was treated with Xiyanping injection combined with azithromycin for the first day and no abnormalities were observed. After the second administration of Xiyanping injection for 1 h, there was a scattered rash on the head, face and neck, without any itching sensation, and the body temperature was 38 ℃. After eliminating effects of food and other drugs, Xiyanping injection was stopped immediately. After the deep red rash increased, there were new rashes on the chest, back and limbs, and anti-allergic treatment with diphenhydramine hydrochloride injection was treated;the rash continued to expand before the fusion, and the fusion was in the form of a sheet, the body temperature was 39.7 ℃. And it was considered to be polymorphic erythema, the patient was given methylprednisolone sodium phosphate, calcium gluconate injection, Ibastine tablets and Ketotifen fumarate tablets, physical cooling was conducted;after treatment, the symptoms were improved. The adverse drug reaction were most likely induced by Xiyanping injection. Traditional Chinese medicine injections are complex in composition and contain a variety of allergenic ingredients. Therefore, it should be used with caution in elderly patients, especially those with high sensitivity.
