摘要
目的分析妊娠期高血压病的影响因素及预防方法。方法选取于2017年5月至2018年5月就诊于我科的500例孕妇,分为妊高病组100例和正常孕妇组400例,统计并分析两组相关资料并分析影响因素。结果高龄(年龄≥40岁)、初次产检时体质量指数(BMI)≥35 kg/m2、孕早期高血压、高血压家族病史均明显高于对照组(t=3.14、2.59、13.42、13.54,P<0.01),高龄(年龄≥40岁)、初次产检时BMI≥35 kg/m2、孕早期高血压、高血压家族病史、子痫前期家族病史是妊高症的危险因素(分别对应OR=2.84、4.76、2.55、2.79,P<0.05),孕前、孕后服用阿司匹林抗凝治疗、适度锻炼与合理作息是妊高症的保护性因素(分别对应OR=0.79、0.69,P<0.05)。结论高龄(年龄≥40岁)、初次产检时BMI≥35 kg/m2、孕早期高血压、高血压家族病史、子痫前期家族病史是妊高症的危险因素;孕前、孕后服用阿司匹林抗凝治疗、适度锻炼与合理作息是妊高症的保护性因素;临床产科医生应该引起足够重视。
Objective To analyze the influencing factors and prevention methods of hypertension complicating pregnancy.Methods 500 pregnant women from May 2017 to May 2018 were divided into two groups:100 cases of PIH and 400 cases of normal pregnant women.The related data of the two groups were statistically analyzed and the influencing factors were analyzed.Results The elderly(age≥40 years old,BMI≥35 kg/m^2 at first birth examination,hypertension in early pregnancy,family history of hypertension)were significantly higher than those in control group(t=3.14,2.59,13.42,13.54,P<0.01),and the old(age≥40 years old,first delivery)were significantly higher than those in the control group.BMI≥35 kg/m^2,hypertension in early pregnancy,family history of hypertension and family history of preeclampsia were risk factors of PIH(OR=2.84,4.76,2.75,2.79,P<0.05).Before and after pregnancy,aspirin was used as anticoagulant therapy.Moderate exercise and reasonable working and rest were the protective factors of PIH(OR=0.79,0.69,P<0.05).Conclusion The elderly(age≥40 years),BMI≥35 kg/m2,hypertension in early pregnancy,family history of hypertension and family history of preeclampsia are pregnancy induced hypertension.Risk factors of the disease;Aspirin anti-coagulation therapy,moderate exercise and reasonable interest rate were the protective factors of pregnancy induced hypertension.Clinicians should pay enough attention.
