摘要
目的促进临床合理使用抗菌药物,优化医院药事管理。方法随机抽取2015年6月和2016年6月镇江地区15所二级医院的门(急)诊处方和住院病历,现场调查抗菌药物临床应用管理情况,并进行统计和分析。结果 2015年和2016年,镇江地区二级医院住院患者抗菌药物平均使用率由(52. 89±21. 60)%降至(51. 31±17. 01)%,平均使用强度由(45. 75±18. 12) DDD/(100人·天)降至(44. 67±22. 10) DDD/(100人·天),门诊患者抗菌药物平均使用率由20. 10%降至13. 94%,急诊患者抗菌药物平均使用率由35. 76%降至32. 88%;Ⅰ类切口手术预防使用抗菌药物比例及原则上不预防使用抗菌药物而实际预防使用的比例均降低;抗菌药物品种选择平均合理率升高,而用药时机和用药疗程的平均合理率均有所下降。结论镇江地区二级医院抗菌药物临床应用管理成效显著,整体发展趋势良好,但也存在一些问题,仍需加强管理。
Objective To promote the rational use of antibiotics in the clinic and optimize the pharmacy administration in the hospital.Methods The outpatient and emergency prescriptions and inpatient medical records were selected randomly in 15 secondary hospitals in June 2015 and June 2016 in Zhenjiang.The clinical application and management of antibiotics were investigated on-site,the statistics and analysis were carried out.Results In 2015 and 2016,the average usage rate of antibiotics in inpatients of secondary hospitals in Zhenjiang decreased from(52.89±21.60)% to(51.31±17.01)%,and the average usage intensity decreased from(45.75±18.12)DDD/(100 people·day)to(44.67±22.10)DDD/(100 people·day).The average use rate of antibiotics for outpatients decreased from 20.10% to 13.94%,which decreased from 35.76% to 32.88% for emergency patients.In type I incision surgery,the ratio of prophylactic use of antibiotics and the ratio of actual prophylactic use without prophylactic use of antibiotics in principle were decreased.The average rational rate of antibiotics categories selection was increased,while the average rational rates of medication timing and medication course were decreased.Conclusion The clinical application and management of antibiotics in secondary hospitals in Zhenjiang were effective.The overall development tendency is good,but there are still some problems,and the administration still need to be strengthened.
