摘要
目的探讨基于早期预警评分系统(PEWS)的分级管理在儿科重症监护病房(PICU)护理质量中的应用效果。方法将2016年1—12月收治629例危急重症患儿未实施早期预警评分系统的分级管理设为实施前组,将2017年1—12月收治的738例危急重症患儿实施早期预警评分系统的分级管理设为实施后组。比较实施前后护理质量指标评分、护理不良事件发生情况及医师、护士、患儿家属满意度。结果实施后组PICU护理安全管理、基础护理、病区管理、感染管理护理质量指标评分均明显高于实施前组(P <0. 05),两组护理文书书写和急救管理比较差异无统计学意义(P> 0. 05);实施后组PICU患儿呼吸机相关性肺炎(VAP)、导管相关性血流感染(CRBSI)、导管相关性尿路感染(CAUTI)、意外拔管、压疮、坠床不良事件发生率明显低于实施前组(P <0. 05);实施后组医师、护士及患儿家属满意度高于实施前组(P <0. 05)。结论基于PEWS评分的分级管理,可明显提高PICU护理质量,降低护理不良事件发生,提高医护人员及家属满意度。
Objective To explore the application effect of hierarchical management based on pediatric early warning score(PEWS)system in nursing quality of pediatric intensive care unit(PICU). Methods To select 629 cases of critically ill children who didn't have hierarchical management of PEWS system in our hospital from January 2016 to December 2016 as the pre-implementation group. A total of 738 cases of critically ill children who had hierarchical management of PEWS system in our hospital from January 2017 to December 2017 were set as the post-implementation group. The scores of nursing quality indicators, the occurrence of adverse nursing events and the satisfaction of physicians, nurses and family members before and after the implementation of management were compared. Results The nursing safety management, basic nursing, ward management, and infection management nursing quality index scores of the post-implementation group were significantly higher than that in the pre-implementation group( P <0.05). There was no statistically significant difference between the two groups of nursing document writing and emergency management( P >0.05). The incidence of ventilator associated pneumonia(VAP), catheter related blood stream infection(CRBSI), catheter-associated urinary tract infections(CAUTI), accidental extubation, pressure sore, and bed-drop adverse events of PICU children in the post-implementation group was significantly lower than that of the pre-implementation group( P <0.05). The satisfaction of doctors, nurses, and families in the post-implementation group was higher than that of the pre-implementation group( P <0.05). Conclusion The hierarchical management based on PEWS system can increased the quality of PICU nursing significantly. Meanwhile, it can reduce the incidence of nursing adverse events, and improve the satisfaction of medical staff and family members.
