摘要
目的观察奥利司他联合阿托伐他汀治疗单纯性肥胖并高脂血症的效果。方法将150例单纯性肥胖并高脂血症患者随机分为观察组和对照组,每组75例。观察组给予奥利司他联合阿托伐他汀治疗,对照组给予阿托伐他汀治疗。疗程均为3个月。比较两组患者临床疗效、体重、体质指数(BMI)、腰围及TC、TG、HDL-C、LDL-C、载脂蛋白(Apo) A1、Apo B-100、血清内皮素-1(ET-1)、一氧化氮、鸢尾素、超敏C反应蛋白(hs-CRP)、肿瘤坏死因子(TNF)-α、基质金属蛋白酶-9(MMP-9)水平及不良反应发生情况。结果治疗后,观察组总有效率高于对照组(P <0. 05);两组患者TC、TG、LDL-C、Apo B-100、ET-1水平均低于治疗前,HDL-C、Apo A1、一氧化氮、鸢尾素水平高于治疗前,且观察组上述指标水平改善优于对照组(均P <0. 05);两组患者体重、BMI、腰围及hs-CRP、TNF-α、MMP-9水平低于治疗前,且观察组上述指标水平均低于对照组(均P <0. 05)。两组患者不良反应发生率差异无统计学意义(P> 0. 05)。结论奥利司他联合阿托伐他汀治疗单纯性肥胖并高脂血症可降低患者的体重,并改善血脂水平及血管内皮功能。
Objective To observe the effect of orlistat combined with atorvastatin on simple obesity complicated with hyperlipidemia.Methods A total of 150 patients with simple obesity and hyperlipidemia were randomly divided into observation group and control group,with 75 cases in each group. The observation group was treated with orlistat combined with atorvastatin for 3 months,and the control group was treated with atorvastatin for 3 months. The indices of the two groups were compared,including clinical efficacy,body weight,body mass index( BMI),waist circumference,the levels of TC,TG,HDL-C,LDL-C,apolipoprotein( Apo) A1,Apo B-100,serum endothelin-1( ET-1),nitric oxide,irisin,high-sensitivity C-reactive protein( hs-CRP),tumor necrosis factor( TNF)-α and matrix metalloproteinase-9( MMP-9),and the incidence of adverse reactions. Results After treatment,the total effective rate of the observation group was higher than that of the control group( P < 0. 05); the levels of TC,TG,LDL-C and Apo B-100,ET-1 were lower while the levels of HDL-C Apo A1,nitric oxide and irisin were higher in both groups in contrast with those before treatment,and the improvement in the levels of the indices described above of the observation group was superior to that of the control group( all P < 0. 05); body weight,BMI,waist circumference and the levels of hs-CRP,TNF-α and MMP-9 were lower in both groups compared with those before treatment,and the levels of the indices described above of the observation group were lower than those of the control group( all P < 0. 05). There was no statistically significant difference in the incidence rate of adverse reactions between the two groups( P > 0. 05). Conclusion Orlistat combined with atorvastatin can reduce body weight and improve blood lipid and vascular endothelial function in the treatment of patients with simple obesity complicated with hyperlipidemia.
