摘要
目的观察腰麻下行手法松解术联合中药外洗治疗创伤性膝关节强直症的临床效果。方法将130例创伤性膝关节强直症患者随机分为对照组及观察组,每组65例。两组患者均在腰麻下行手法松解术,术后应用持续被动训练(CPM)仪进行功能锻炼,观察组在此基础上加用中药外洗,治疗15 d,随访6个月。采用Judet膝关节功能评分评价患者膝关节功能,对比治疗前及治疗后6个月患者膝关节功能Lysholm评分,观察治疗后6个月复发情况。结果观察组患者的膝关节功能优良率为93. 85%,高于对照组的76. 92%(P <0. 05)。治疗后两组患者膝关节功能Lysholm评分中的跛行评分、疼痛评分、肿胀评分均高于治疗前,并且观察组上述指标评分高于对照组(均P <0. 05)。观察组的复发率低于对照组(P <0. 05)。结论腰麻下行手法松解术联合中药外洗治疗创伤性膝关节强直症临床效果较好,复发率低。
Objective To observe the clinical efficacy of therapeutic manipulation for arthrolysis under spinal anesthesia combined with external washing using traditional Chinese medicine on traumatic ankylosis of knee joint. Methods A total of 130 patients with traumatic ankylosis of knee joint were randomly divided into control group and observation group,with 65 cases in each group. Both groups underwent therapeutic manipulation for arthrolysis under spinal anesthesia combined with postoperative functional training using the continuous passive motion( CPM) apparatus,in addition,the observation group received external washing using traditional Chinese medicine,and the treatment lasted for 15 days prior to a 6-month follow-up. The Judet knee joint function score was used to assess the patients’ knee joint function,the Lysholm knee joint scores of the patients before treatment and after 6 months of treatment were compared,and the recurrence at 6 months after treatment was observed. Results The excellent and good rate of the patients for knee joint function in the observation group was higher than that in the control group( 93. 85% vs. 76. 92%,P < 0. 05). After treatment,the Lysholm knee joint scores of the patients,including the scores for lameness,pain and swelling domains,were higher in both groups compared with the scores before treatment,and the scores for the indices described above in the observation group were higher than those in the control group( all P < 0. 05). The recurrence rate in the observation group was lower than that in the control group( P < 0. 05). Conclusion The use of therapeutic manipulation for arthrolysis under spinal anesthesia combined with external washing using traditional Chinese medicine achieves better clinical efficacy and a low recurrence rate in treating traumatic ankylosis of knee joint.
