体外冲击波碎石和输尿管镜钬激光碎石治疗输尿管上段结石疗效对比 被引量:37

摘要 目的对比体外冲击波碎石(ESWL)和经输尿管镜钬激光碎石(URL)治疗输尿管上段结石的疗效。方法回顾性收集2016年1至2017年12月在我院接受治疗的输尿管上段结石患者共982例,选择体外冲击波碎石患者580例,选择输尿管镜钬激光碎石患者402例,将两组碎石成功率、并发症进行比较研究。结果结石直径> 1 cm,URL组的总体碎石成功率高于ESWL组(85.4%vs 60.7%),并发症率低于ESWL组(6.1%vs 15.4%),差异具有统计学意义(P <0.05);结石直径≤1 cm,URL组的总体碎石成功率与术后发症率与ESWL组相似,差异无统计学意义(P> 0.05)。结论对于输尿管上段大于1 cm的结石,URL较ESWL更安全、有效;而对于输尿管上段小于等于1 cm的结石,两种治疗方法疗效相似。一水草酸钙、磷酸钙结石质地较硬,更适宜选择行URL。 Objective To compare the efficacy of extracorporeal shock wave lithotripsy(ESWL)versus ureteroscopic holmium laser lithotripsy(URL)for upper ureteral calculi.Methods A total of 982 cases of upper ureteral calculi treated in our hospital from Jan. 2016 to Dec. 2017 were enrolled. 580 cases received ESWL and 402 cases received URL. The success rate and complications of the two groups were compared and the compositions of calculi were analyzed.Results For calculi > 1 cm,the success rate of group URL was higher than that of group ESWL(85.4% vs. 60.7%),and the rate of complication was lower than that of group ESWL(6.1% vs. 15.4%). The difference was statistically significant(P < 0.05);and for calculi < 1 cm. the success rate of the group URL was similar to that of the ESWL group,and there was no significant difference statistically(P > 0.05).Conclusion URL is safer and more effective than ESWL for the upper ureteral calculi > 1 cm,and the two therapy are similar for the upper ureteral calculi < 1 cm. Considering calcium oxalate monohydrate and calcium phosphate calculi are hard,so they are more suitable to receive URL.
