摘要
目的对比自动痔疮套扎和传统内扎外切术治疗中重度混合痔患者的临床效果。方法选取2017年1月至2018年1月新疆医科大学附属中医医院肛肠科收治的98例中重度混合痔患者为研究对象,按照治疗方式的不同分为对照组(48例)和观察组(50例)。对照组行传统内扎外切术治疗,观察组行自动痔疮套扎联合外切术治疗。对比两组患者的临床疗效,各项手术指标及并发症发生情况。结果观察组总有效率高于对照组[96. 0%(48/50)比81. 3%(39/48)](P <0. 05)。观察组患者的手术时间、创面愈合时间及术后住院时间均明显短于对照组[(13. 5±2. 1) min比(19. 3±2. 6) min、(14. 2±2. 0) d比(18. 8±2. 1) d、(5. 7±1. 0) d比(7. 7±1. 1) d](P <0. 01),术中出血量明显少于对照组[(15. 8±1. 9) m L比(26. 7±2. 3) m L](P <0. 01)。观察组患者术后肛门疼痛、切口水肿、尿潴留、排尿障碍发生率低于对照组(P <0. 05)。治疗后3 d观察组视觉模拟评分低于对照组[(2. 8±0. 4)分比(3. 9±0. 7)分](P <0. 01)。结论采用自动痔疮套扎联合外切术治疗中重度混合痔的效果明显优于传统内扎外切术,且安全性较高。
Objective To compare the clinical effects of automatic hemorrhoids ligation and traditional internal ligation in the treatment of patients with moderate to severe mixed hemorrhoids.Methods A total of98patients with moderate to severe mixed hemorrhoids who were admitted to Affiliated Traditional Chinese Medicine Hospital of Xinjiang Medical University from Jan.2017to Jan.2018were enrolled in the study,and were divided into a control group(48cases)and an observation group(50cases)according to the treatment method.The control group underwent traditional internal ligation and external excision.The observation group was treated with automatic hemorrhoids ligation combined with external excision.The clinical efficacy,various surgical indexes and complications were compared between the two groups.Results The total effective rate of the observation group was higher than that of the control group[96.0%(48/50)vs81.3%(39/48)](P<0.05).The operative time,wound healing time and postoperative hospitalization time in the observation group were significantly shorter than those in the control group[(13.5±2.1)min vs(19.3±2.6)min,(14.2±2.0)d vs(18.8±2.1)d,(5.7±1.0)d vs(7.7±1.1)d](P<0.01),the amount of intraoperative bleeding was significantly less than that of the control group[(15.8±1.9)mL vs(26.7±2.3)mL](P<0.01).The incidence of postoperative anal pain,incision edema,urinary retention and dysuria in the observation group were lower than those in the control group(P<0.05).The visual analogue scale of the observation group3days after treatment was lower than that of the control group[(2.8±0.4)score vs(3.9±0.7)score](P<0.01).Conclusion The effect of automatic hemorrhoids ligation combined with external excision for moderate to severe mixed sputum is better than the traditional internal ligation,and has high safety.
