摘要
目的系统评价高b值弥散加权成像(DWI)诊断前列腺癌的准确性。方法检索1995年1月1日至2018年5月31日Pub Med、EMBASE、Cochrane Library、中国生物医学文献数据库、万方数据、维普、中国期刊全文数据库相关文献,按照纳入标准筛选关于高b值DWI对前列腺癌诊断的研究。采用诊断试验质量评价工具QUADAS-2标准对文献进行质量评估,采用Meta-DiSc 1. 4软件检验研究间的异质性及汇总分析,Stata 14. 0软件测定发表偏倚。结果共纳入16篇符合标准的文献,高b值DWI对前列腺癌诊断的灵敏度为0. 83(95%CI 0. 81～0. 85),特异度为0. 91(95%CI 0. 90～0. 92),阳性似然比为7. 57(95%CI 5. 61～10. 20),阴性似然比0. 19(95%CI0. 16～0. 23),诊断优势比46. 80(95%CI 34. 80～62. 94)。汇总受试者工作特征曲线下面积为0. 935 1。结论高b值DWI诊断前列腺癌具有较高的敏感性和特异性,对诊断和鉴别诊断前列腺癌具有很高的临床诊断价值。
Objective To systematically evaluate the diagnostic accuracy of high b-value diffusion weighted imaging for prostate cancer.Methods Computer-based search of PubMed,EMBASE,Cochrane Library,CBM,Wanfang,VIP database and CNKI databases from Jan.1st,2015to May31st,2018were retrieved.The diagnostic studies about high b-value diffusion weighted imaging for prostate cancer were screened according to the inclusion criteria,and the quality of the literature was assessed using the diagnostic test quality evaluation tool QUADAS-2standard.Metadisc1.4software was used for heterogeneity examination and Meta-analysis.Stata14.0software was used to evaluate publication bias.Results Totally16studies met the inclusion criteria.The results of the Meta-analysis showed that the pooled sensitivity of the diagnosis of high b-value diffusion weighted imaging for prostate cancer was0.83(95%CI0.81-0.85),the pooled specificity was0.91(95%CI0.90-0.92),the pooled positive likelihood ratio was7.57(95%CI5.61-10.20),the pooled negative likelihood ratio was0.19(95%CI0.16-0.23),the pooled OR was46.80(95%CI34.80-62.94),and the pooled summary receiver operating characteristics area under the curve was0.9351.Conclusion High b-value diffusion weighted imaging is of high sensitivity and specificity in the diagnosis of prostate cancer.It has a high clinical value for the diagnosis and differential diagnosis of prostate cancer.
