摘要
目的观察紫杉醇+奈达铂新辅助化疗联合同期放化疗治疗中晚期鼻咽癌的临床疗效。方法将60例中晚期鼻咽癌患者平均分为观察组与对照组,每组30例。对照组采用同期放化疗治疗,观察组在对照组基础上另给予紫杉醇+奈达铂新辅助化疗治疗。治疗后,评价2组的近期疗效。治疗期间,观察患者出现的的毒副作用。治疗结束后,对所有患者进行2年的电话随访,观察患者的1年生存率、2年生存率、局部复发率及远处转移率。结果观察组治疗总有效率为86. 7%,显著高于对照组的63. 3%(P <0. 05)。治疗期间,观察组腹痛腹泻、恶心呕吐、血细胞异常、口腔黏膜炎、放射性中耳炎及肝肾功能损伤的发生率分别为26. 7%、43. 3%、13. 3%、16. 7%、13. 3%及6. 7%,与对照组均基本一致(P> 0. 05)。观察组1年生存率及2年生存率分别为93. 33%、76. 67%,均明显高于对照组;而观察组局部复发率及远处转移率分别为13. 33%、6. 67%,均明显低于对照组(P <0. 05)。结论紫杉醇+奈达铂新辅助化疗联合同期放化疗治疗中晚期鼻咽癌疗效较好,可有效延长患者生存时间,值得进行深入探讨。
Objective To study the clinical effect of taxol+nedaplatin neoadjuvant chemotherapy combined with concurrent radiotherapy and chemotherapy for middle and advanced stage nasopha ryngeal carcinoma.Methods 60 patients with middle and advanced stage nasopha ryngeal carcinoma in our hospital were divided into observation and control group,30 cases in each group.The control group was treated with by concurrent radiotherapy and chemotherapy while the observation group was given taxol+nedaplatin neoadjuvant chemotherapy on the base of control group.After the treatment,the effects were observed in the 2 groups.During the treatment,toxic reaction rates were observed.After the treatment,2 years followed up were performed in all patients,1 year survival rate,2 year survival rate,local recurrent rate and distant metastasis rate were observed.Results In observation group,the total effective rate were 86.7%,significantly higher than 63.3%in control group(P<0.05).During the treatment,the rated of abdominal pain and diarrhea,nausea and vomiting,abnormal blood cells,oral mucosal inflammation,radiation otitis and liver function injuries were 26.7%,43.3%,13.3%,16.7%,13.3%and 6.7%,similar with the control group(P>0.05).In the 2 years follow-up,1 year survival rate and 2 year survival rate were 93.33%and 76.67%,significantly higher than control group(P<0.05),local recurrent rate and distant metastasis rate were 13.33%and 6.67%,significantly lower than control group(P<0.05).Conclusion The method of taxol+nedaplatin neoadjuvant chemotherapy combined with concurrent radiotherapy and chemotherapy for middle and advanced stage nasopha ryngeal carcinoma has a excellent effects,can improve the survive time and worth to the next study.
