摘要
目的探讨老年冠心病患者不同冠脉病变程度与内皮祖细胞CD34^+相关性。方法 127例老年患者根据冠脉造影结果分为冠心病组91例和对照组36例。根据ACC/AHA冠脉病变形态分型将冠心病组患者分为A型、B型、C型病变组;根据SYNTAX积分将冠心病组患者分为低危组、中危组、高危组;根据Gensini积分将冠心病组患者分为低分组、中分组、高分组。比较冠心病组与对照组实验室各检查指标,并比较不同冠状动脉病变形态、不同SYNTAX积分及不同Gensini积分冠心病患者CD34^+水平。冠心病组血清CD34^+水平Gensini积分进行相关性分析。结果冠心病组CD34^+百分率明显低于对照组(P <0. 05)。冠脉病变形态分型中C型病变组CD34^+百分率低于A型病变组(P <0. 05),B型病变组CD34^+百分率与A型病变、C型病变组比较差异无统计学意义。SYNTAX积分中高危组CD34^+百分率低于低危组(P <0. 05),中危组CD34^+百分率与高危组、低危组比较差异无统计学意义。Gensini积分中高分组CD34^+百分率低于低分组(P <0. 05),中分组CD34^+百分率与高分组、低分组比较差异无统计学意义。相关分析结果显示,血清CD34^+百分率与老年冠心病患者Gensini积分呈负相关(r=-0. 378,P <0. 01)。结论老年冠心病患者CD34^+水平降低与冠脉病变支数及狭窄程度相关,检测外周血CD34^+水平可在一定程度上反映冠脉病变程度。
To investigate the correlation between the severity of coronary artery lesions in elderly patients and peripheral blood cell CD34+ratio.One hundred and twenty-seven elderly patients were divided into coronary heart disease group(n=91)and normal control group(n=36)according to the result of coronary angiography.According to the morphological classification of ACC/AHA coronary lesions,the coronary heart disease group was further divided into type A lesions,type B lesions and type C lesions.According to SYNTAX score,the patients were divided into low risk group,moderate risk group and high-risk group.According to Gensini score,the patients were divided into low score group,middle score group and high score group.The various laboratory indexes were compared between the coronary heart disease group and the control group.The levels of CD34+cells among the different coronary artery lesions,the different SYNTAX scores and the different Gensini scores were compared.The correlation between serum CD34+level and Gensini score in patients with coronary heart disease was analyzed by Pearson correlation analysis.The percentages of CD34+cells in the coronary heart disease group were significantly lower than those in the normal control group(P<0.05).The percentages of CD34+cells in the type C lesion group was lower than that in the type A lesion group(P<0.05)but the type B group was not significantly different from the type A group.The percentages of CD34+cells in the high Gensini score group was lower than that in the low score group(P<0.05),but the middle group was not significantly different from the high and low groups.The correlation analysis showed that the percentage of serum CD34+was negatively correlated with the Gensini score(r=-0.378,P<0.01).The decrease of levels of CD34+cells in elderly patients with coronary heart disease is related to number of coronary artery disease branches and degree of stenosis.Detection of peripheral blood levels of CD34+cells can reflect the extent of coronary lesions to a certain extent.
