摘要
目的研究Tourette综合征(Tourette syndrome,TS)患儿由韦氏儿童智力量表评价的认知功能情况,并探讨阿立哌唑治疗对其的影响。方法纳入TS患儿30例为TS组,健康儿童30名为对照组,入组时对两组进行韦氏儿童智力量表评分,TS组患儿给予阿立哌唑治疗6个月后再次评价韦氏儿童智力量表评分,并收集患儿年龄、发病年龄、病程、耶鲁综合抽动严重程度量表(Yale global tic severity scale, YGTSS)评分等可能的影响因素。结果 TS组治疗前与对照组相比,算术、编码、填图、操作智商评分较低,差异有统计学意义(P<0.05);其余各项在治疗前后两组间差异均无统计学意义(P>0.05)。TS组治疗后与治疗前相比,填图评分提高,差异有统计学意义(P<0.05)。线性回归分析示,患儿治疗前言语智商(β=-0.647,P=0.002)、操作智商(β=-0.612,P=0.001)及总智商(β=-0.798,P<0.001)与YGTSS评分分别呈负关联。结论 TS患儿韦氏儿童智力量表评分在正常范围,但存在评分结构不均衡,YGTSS评分可能是影响其评分的危险因素。阿立哌唑对TS患儿韦氏儿童智力量表评分无明显影响。
Objective The effects of aripiprazole on the cognitive functioning in children with Tourette Syndrome(TS)was assessed using Chinese Wechsler Intelligence Scale for Children(C-WISC).Method Thirty children with TS were included in the TS group and 30 healthy children served as the control group.The age,age of onset,duration of disease and the Yale Global Tic Severity Scale(YGTSS)scores were collected.All children were assessed using C-WISC at the beginning.Children in the TS group was re-evaluated after 6 months of treatment with aripiprazole.Results Compared with the control group,children in TS group had significantly lower scores in the arithmetic,coding,mapping,and performance intelligence quotient(P<0.05).There were no significant differences between the groups with respect to other indicators before and after the treatment(P>0.05).The score of mapping in TS group was significantly improved after treatment compared with before treatment(P<0.05).Multivariate linear regression analysis showed that the verbal intelligence quotient(β=-0.647,P=0.002),performance intelligence quotient(β=-0.612,P=0.001)and full scale intelligence quotient(β=-0.798,P<0.001)of TS children before treatment were negatively correlated with YGTSS score,respectively.Conclusion The score of C-WISC with TS is in the normal range,but there is imbalance in grading structure.The score of YGTSS may be a risk factor in assessment of the C-WISC.Aripiprazole does not have any significant effect on the C-WISC score of children with TS.
