妊娠期糖尿病孕妇产前综合干预及产后随访情况分析 被引量:8

摘要 目的探讨妊娠期糖尿病(GDM)孕妇产前综合干预对妊娠结局和血糖控制的影响。方法将我院产科门诊确诊的264例GDM孕妇按住院顺序分为实验组和对照组,两组均采用常规的治疗和护理措施,实验组在此基础上还进行产前综合干预,并随访至产后1年。采用非同期临床对照研究的方法,比较两组孕妇的妊娠结局和血糖控制情况。结果实验组妊娠期高血压、产后出血及新生儿窒息发生率均低于对照组,产后随访42天、半年及1年孕妇的糖代谢异常率均低于对照组,差异有统计学意义。结论重视GDM的筛查,采取积极的综合干预措施,能有效改善妊娠结局,还能促进产后血糖指标的控制。 Objective To investigate the effect of comprehensive prenatal intervention on pregnancy outcomes and postnatal impaired glucose regulation(IGR)prevalence in women with gestational diabetes mellitus(GDM).Methods In this study,264 women diagnosed with GDM in obstetric out-patient clinic were divided into intervention group or control group according to admission sequence.All patients received conventional therapy and nursing care throughout the study and were followed up for 1 year after delivery.The intervention group,in addition,received comprehensive prenatal intervention.Endpoints were pregnancy outcomes and postnatal IGR prevalence.Results As compared with control group,prevalence of pregnancyinduced hypertension,postpartum hemorrhage and neonate asphyxia was significantly decreased in intervention group.Meanwhile,lower postnatal IGR prevalence was observed in intervention group.Conclusion GDM screening and comprehensive prenatal intervention can effectively improve pregnancy outcomes and postnatal glucose metabolism.
