摘要
目的探讨腹部B超筛查在胎儿畸形中的应用价值。方法对2014年9月至2017年8月在我院行产前检查和分娩的孕妇3 427例行产前腹部B超筛查,与胎儿引产的尸检结果或新生儿检查结果进行比较,分析产前B超筛查的准确率、灵敏度、特异度、阳性预测值、阴性预测值及不同孕周孕妇胎儿畸形检查检出结果。结果 3 427例胎儿中有58例畸形胎儿,产前B超筛查检出53例,准确率为91.4%;胎儿B超筛查结果与术后解剖结果比较,差异无统计学意义(P>0.05)。腹部B超筛查的灵敏度为91.4%、特异度为100.0%、阳性预测值为100.0%、阴性预测值为99.9%。孕中期胎儿畸形检出率达69.8%;孕中期孕妇胎儿畸形的检出率高于孕晚期,差异具有统计学意义(P<0.05)。结论产前胎儿B超筛查的准确率较高,并且最好在孕中期进行胎儿畸形检查,值得临床推广和应用。
bjective To explore the application value of abdominal B-ultrasonography screening in fetal malformation.M e t h o d s From September 2014 to August 2017,3 427 pregnant women of prenatal examination and delivery in our hospital took abdominal B-ultrasonography screening,then the results and the fetus induction of autopsy results or neonatal examination results were compared,and the accuracy,sensitivity,specificity,positive predictive value,negative predictive value and the detection of fetal malformations in different gestational weeks pregnant women were analyzed.R e s u l t s Among 3 427 fetuses,58 fetuses had malformation,53 cases were detected by abdominal B-ultrasonography screening,with an accuracy rate of 91.4%.There was no statistically significant difference between abdominal B-ultrasonography screening results and postoperative anatomy results(P>0.05).Abdominal B-ultrasonography screening had sensitivity of 91.4%,specificity of 100.0%,positive predictive value of 100.0%,negative predictive value of 99.9%.The detection rate of fetal malformations in the second trimester was 69.8%.The detection rate of fetal malformation in the second trimester was higher than that in the third trimester,the difference was statistically significant(P<0.05).C o n c l u s i o n The accuracy of abdominal B-ultrasonography screening is high,and fetal malformations examination in the second trimester is the best,it is worthy of clinical application and promotion.
