摘要
目的研究食管癌患者术后早期全肠内营养的临床治疗结果。方法将262例食管癌患者按照随机数字表法分为全肠内营养组131例和常规肠外全静脉营养组131例。比较两组患者营养支持前后的安全性指标、营养指标、免疫指标和急性炎性指标,观察术后并发症、住院时间等。结果所有患者在研究期间无死亡,无严重并发症,无明显肝、肾功能改变;研究结束时,全肠内营养组血清前清蛋白、外周血淋巴细胞总数明显高于肠外全静脉营养组,IL-6、TNF-α和CRP明显低于肠外全静脉营养组;全肠内营养组住院时间明显短于肠外全静脉营养组。结论全肠内营养与肠外全静脉营养相比,有利于恢复患者术后胃肠道功能,改善患者的营养状况,降低炎性反应水平,平衡免疫功能,减少术后并发症得发生,缩短住院时间,减少住院费用。
Objective To investigate the effect of early enteral nutrition support in postoperative patients with esophageal cancer.Methods 262 cases were divided into two groups with a random number table:the early enteral nutrition group(EEN)(n=131)and the isocaloric total parenteral nutrition through central vein(TPN)(n=131).All variables were measured before and after nutritional supportoperation that includes safety index,nutritional index,immune index and acute inflammatory index;postoperative complications,hospital stay were observed.Results Tolerance of early enteral nutrition was excellent.Neither mortality nor serious morbidity occurred in all patients during the period of study.After the study,the levels of serum prealbumin,peripheral total lymphocytes were significantly higher in the EEN group than the levels in the TPN group.The concentrations of C-reactive protein,IL-6,TNF-αand CRP were significantly lower in the EEN group than that in the TPN group.The length of hospital stay was shorter in EEN group.Conclusion The early enteral nutrition may be more beneficial to the recovery of patients after surgery than TPN.
