胆囊胆固醇结石患者胆道菌群结构、免疫水平与其结石形成的关联研究 被引量:1

摘要 目的探究胆囊胆固醇结石患者胆道菌群结构、免疫水平与其结石形成的关联。方法筛选在武陟县人民医院接受治疗的胆囊胆固醇结石患者76例作为实验组,选取同时期在本院体检的健康者58例作为对照组。利用半定量PCR的方法检测两组研究对象胆汁和胆囊黏膜细菌DNA的阳性率和菌落数,分析菌群种类,并测定胆汁和胆囊黏膜中IgA、IgG和IgM的含量。结果两组研究对象的细菌DNA检出率和细菌菌落数均无显著性差异(P>0.05)。实验组患者感染的细菌主要有大肠杆菌、金黄色葡萄球菌、链球菌、痤疮丙酸杆菌、脆弱类杆菌等,对照组细菌DNA阳性者感染的细菌也在上述范围内,但种类少于实验组。实验组患者胆囊黏膜和胆汁中的IgA、IgG含量显著高于对照组患者(P<0.05),IgM含量无显著性差异(P>0.05)。结论胆囊胆固醇结石患者与健康者菌群结构类似,存在的细菌不足以证明与结石形成相关,但患者的免疫球蛋白水平升高可能与结石形成有关。 Objective To explore the relationship with the structure of flora,the level of immunization and the formation of stones in the patients with gallbladder cholesterol calculus.Methods76patients with gallbladder cholesterol stones treated in our hospital were selected as the experimental group.58healthy subjects at the same time in our hospital were selected as the control group.The positive rate and colony count of DNA in bile and gallbladder mucosa of the two groups were detected by semi quantitative PCR.The species of bacteria were analyzed,and the contents of IgA,IgG and IgM in bile and gallbladder mucosa were determined.Results There was no significant difference in the detection rate of bacterial DNA and the number of bacterial colonies between the two groups(P>0.05).The bacteria in the experimental group were mainly Escherichia coli,Staphylococcus aureus,Streptococcus,Propionibacterium acnes,Bacteroides fragilis and so on.The bacteria infected with DNA positive bacteria in the control group were also in the above range,but the kinds of bacteria were less than those in the experimental group.The contents of IgA and IgG in the gallbladder mucosa and bile of the experimental group were significantly higher than those in the control group(P<0.05),and there was no significant difference in the content of IgM(P>0.05).Conclusion The structure of bacteria in cholesterol gallstone patients was similar to that in healthy persons.The presence of bacteria was not enough to prove that it was related to the formation of stones,but the increase of immunoglobulin level might be related to the formation of stones.
