摘要
目的:探讨中医情志护理对初期血液透析老年患者负性情绪、生活质量及免疫功能的影响。方法:将初期血液透析老年患者60例随机分为观察组与对照组各30例。对照组给予常规护理,观察组患者在常规护理基础上给予中医情志护理。分别应用焦虑自评量表(SAS)和抑郁自评量表(SDS)评定2组患者实施护理前和3个月后的情绪状况;采用生存质量评定量表(WHOQOL-100)评估2组患者生活质量;并检测2组患者外周血T淋巴细胞亚群(CD4^+、CD8^+)以及免疫球蛋白(IgA、IgG、IgM);比较2组患者负性情绪、免疫功能及生活质量。结果:干预前2组患者在情绪评分、生活质量评分和免疫功能方面均无显著性差异(P>0.05);护理干预3个月后,观察组患者SAS及SDS评分较对照组明显降低,差异具有显著性(P<0.01)。观察组患者生理、心理、精神支柱、社会关系和环境、生活质量总评分明显高于对照组,差异具有统计学意义(P<0.05);观察组患者CD4^+细胞计数高于对照组(P<0.01),CD8^+细胞计数低于对照组(P<0.01);观察组患者IgG、IgM水平较对照组增高,差异有统计学意义(P<0.05)。结论:初期老年血液透析患者接受中医情志护理干预能改善患者焦虑抑郁情绪,提高生活质量,增强机体免疫功能。
Objective:To explore the effects of TCM emotional care on negative emotion,quality of life and immunologic function of the elderly patients with incipient hemodialysis.Methods:Sixty patients were randomly separated into the observation group and the control group,30cases each group.The patients in the observation group accepted TCM emotional care on the foundation of routine care,and the control group routine care.Emotional state of the patients before implementing TCM emotional care and in three months were assessed by using SAS and SDS respectively;quality of life of the patients in both groups were evaluated by using WHOQOL-100,and the levels of immunoglobulin(IgA,IgG and IgM)and T lymphocyte subsets(CD4+and CD8+)in peripheral blood of the patients were detected;quality of life,immunologic function and negative emotion of the patients were comparedbetween both groups.Results:The difference had no statistical meaning in the comparisons of emotional scales,the scales of quality of life and immunologic function before the intervention(P>0.05);after intervening by TCM emotional care for three months,SAS and SDS of the patients in the observation group lowered compared with the control group,and the difference showed statistical meaning(P<0.01).The observation group was higher than the control group in total scores of physiology,psychology,spiritual pillar,social relationship,environment,quality of life,and there was a significant difference(P<0.05);the observation group was higher than the control group in CD4+cell counting(P<0.01),lower than the control group in CD8+cell counting(P<0.01);the levels of IgG and IgM of the patients in the observation group elevated compared with the control group,and the difference had statistical meaning(P<0.05).Conclusion:TCM emotional care given to the elderly patients who underwent inicipient hemodialysis could improve the anxiety and depression,raise quality of life and enhance immunologic function.
